温阳利水方联合抗VEGF药物治疗糖尿病视网膜病变黄斑水肿临床观察

摘　　要：目的:观察温阳利水方治疗糖尿病视网膜病变黄斑水肿的临床疗效。方法:选取符合纳入标准的糖尿病视网膜病变黄斑水肿患者42例,随机分为两组各21例。对照组患者眼内注射雷珠单抗,治疗组在对照组基础上口服温阳利水方,治疗的有效起始记录点选择观察到黄斑厚度﹤300μm为标准,记录再次发生黄斑水肿的时间点、黄斑中心视网膜厚度变化、以及各时段的视力改变。结果:在黄斑水肿复发及治疗3w时,比较两组患者的最佳矫正视力,差异均有统计学意义(P﹤0.05),且治疗组疗效优于对照组(P﹤0.05);并且将两组黄斑水肿复发时间进行对比,差异有统计学意义(P﹤0.05),且治疗组黄斑水肿复发时间更迟;两组患者复发时的黄斑中心厚度比较,差异有统计学意义(P﹤0.05),且治疗组复发时黄斑中心厚度低于对照组(P﹤0.05)。结论:温阳利水方联合眼内注射抗VEGF药物对治疗糖尿病性黄斑水肿疗效较好,可延缓其复发及降低黄斑水肿的程度,提高患者视力,减轻黄斑水肿。Objective:To observe the clinical efficacy of Wenyang Lishui formula on diabetes retinopathycaused macular edema.Methods:42 cases of eligible patients with diabetes retinopathy-caused macular edema were selected and randomly divided into two groups,21 cases and 28 eyes in each group.The patients in control group were intraocularly injected ranibizumab,while the patients in treatment group were treated plus with oral Wenyang Lishui formula on the basis of control group.At the effective initial recording point of treatment,the observed macular thickness﹤300μm was selected as the standard.The time point when macular edema occurred again,the changes of retinal thickness in macular center and the changes of vision in each period were recorded.Results:When macular edema recurred and was treated for 3 weeks,the best corrected vision of patients was compared between two groups,the difference was statistically significant(P﹤0.05),and the curative effect of treatment group was better than that of control group(P﹤0.05).The recurrence time of macular edema was compared between two groups,the difference was statistically significant(P﹤0.05),and the recurrence time of macular edema of treatment group was later.The thickness of macular center of patients was compared between two groups during the period of recurrence,the difference was statistically significant(P﹤0.05),and the thickness of macular center of treatment group was smaller than that of control group during the period of recurrence(P﹤0.05).Conclusion:The clinical efficacy of Wenyang Lishui formula and anti-VEGF drugs for intraocular injection on diabetes retinopathy-caused macular edema is better,which can delay its recurrence,reduce the degree of macular edema,improve patients'vision and alleviate macular edema.
