摘  要:目的基于PI3K/AKT通路探讨柴苓汤加味对阴虚热盛代谢综合征-胰岛素抵抗(metabolic syndrome and insulin resistance,MS-IR)大鼠血管内皮功能调控的部分作用机制及病证结合治疗的优势。方法50只SPF级SD雄性大鼠随机分为空白组、造模组。造模组大鼠采用高脂高糖饲料喂养,注射小剂量链脲佐菌素(streptozotocin,STZ)(30 mg/kg)联合中药(干姜、附子)灌服造模,建立阴虚热盛型MS-IR大鼠模型。造模成功后随机分为模型组、柴苓汤加味组(病证结合治疗)、柴苓汤组(因病治疗)、西药组(吡格列酮),各组分别进行柴苓汤加味、柴苓汤、吡格列酮药物干预治疗2周,空白组、模型组给予等体积的蒸馏水灌服。生化检测大鼠空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)水平,计算胰岛素抵抗指数(HOMA-IR);ELISA法检测血清一氧化氮(NO)水平、血管性血友病因子(vWF);Western blot检测腹主动脉血管内皮磷脂酰肌醇3激酶(phosphatidylinositol3-kinase,PI3K)、磷酸化蛋白激酶(phosphorylated-Akt,P-AKT)、磷酸化内皮一氧化氮(phosphorylated-endothelial nitric oxide synthase,P-eNOS)蛋白表达;HE染色切片,光学显微镜观察颈动脉内膜、中膜厚度以及血管管腔结构情况。结果与空白组比较,模型组大鼠FBG、FINS、HOMA-IR水平明显升高,血清NO水平明显降低,vWF水平明显升高(P<0.01),血管内皮结构发生紊乱出现损伤,内皮PI3K、P-AKT、P-eNOS蛋白表达均下调(P<0.05,P<0.01);与模型组比较,柴苓汤加味组、西药组FBG、FINS、HOMA-IR水平明显降低(P<0.01),柴苓汤加味组、柴苓汤组、西药组血清NO水平明显升高、vWF水平明显降低(P<0.05,P<0.01),血管内皮PI3K、P-AKT、P-eNOS蛋白表达均上调(P<0.05,P<0.01);与西药组比较,柴苓汤加味组FBG、FINS、HOMA-IR水平无明显差异,柴苓汤组NO、vWF水平无明显差异,柴苓汤组血管内皮PI3K蛋白表达上调(P<0.05),柴苓汤加味组NO水平明显�Objective To investigate the mechanism of the regulation of MS-IR rats.Methods 50 SPF grade SD male rats were randomly divided into blank group and model group.Modeled rats were fed with high-fat and high-glucose feed,and low-dose STZ(30 mg/kg)combined with traditional Chinese medicine(dried ginger and aconite)was used to establish MS-IR rat model.Successful mold making was randomly divided into model group,Modified Chailing Decoction group(combined with disease syndrome treatment),Chailing Decoction group(treatment due to disease)and Western medicine group(pioglitazone).Each group received Modified Chailing Decoction(柴苓汤加味),ChailingDecoction(柴苓汤)and pioglitazone for 2 weeks,and the blank group and model group gave equal volume of distilled water irrigation.Biochemical detection for rat FBG and FINS levels,HOMA-IR calculation,Elisa method for serum nitric oxide(NO)level,vascular hemophilia factor(vWF);Western blot test for abdominal aortic vascular endothelium PI3K,P-AKT,P-eNOS protein expression,HE stained sections,and optical microscopy.Results Compared with the blank group,FBG,FINS,and HOMA-IR levels in the model group were significantly increased,the serum NO level was significantly decreased,vWF level was significant increased(P<0.01),vascular endothelial structure occurs in disorder and appears to damage,endothelial PI3K,P-AKT,and P-eNOS protein expression were all downregulated(P<0.05,P<0.01);Compared with the model group,the levels of FBG,FINS,HOMA-IR in Chailing Decoction group and Western medicine group decreased significantly(P<0.01),the serum NO level increased and vWF level decreased significantly in Modified Chailing Decoction and Western medicine group(P<0.05,P<0.01),vascular endothelial PI3K,P-AKT,and P-eNOS proteins were all upregulated(P<0.05,P<0.01);Compared with the Western medicine group,FBG,FINS,and HOMA-IR levels in Modified Chailing Decoction group,in the Modified Chailing Decoction group NO and vWF levels in Chailing Decoction group with no significant difference,and PI3K
