基于肠道黏膜屏障探讨益火补土生肌法治疗溃疡性结肠炎研究进展

摘　　要：溃疡性结肠炎(ulcerative colitis,UC)是病变累及结直肠黏膜的一种慢性特发性炎症性疾病,其发病机制尚未明确,但认为与遗传、环境和肠黏膜屏障损伤等因素有关,其中肠黏膜屏障功能受损在UC的发病中占首要地位。脾肾阳虚证是UC的常见和主要证候之一。益火补土生肌法治疗UC已在临床应用中取得了良好的疗效。中医可通过口服中药、灌肠、针灸等方法对受损的肠黏膜进行修复从而治疗UC。该文从生物屏障、机械屏障、化学屏障及免疫屏障四个方面梳理了近年来UC肠黏膜屏障受损的机制研究并对益火补土生肌法理论治疗UC的机制进行综述,该理论通过中药汤剂、针灸、灌肠及其他综合疗法在治疗UC的实验和临床研究中取得进展,这体现了中医治疗UC的辨证思想以及多途径多靶点的优势,也为未来UC的发病机制及治疗的基础和临床研究提供参考。Ulcerative colitis(UC)is a chronic idiopathic inflammatory disease involving colorectal mucosa.The pathogenesis of ulcerative colitis is not yet clear,but it is believed to be related to genetic,environmental and intestinal mucosal barrier damage,among which the impairment of intestinal mucosal barrier function plays a leading role in the pathogenesis of UC.Deficiency of spleen and kidney Yang is one of the common and main syndromes of UC.The treatment of UC by supplementing fire and invigorating soil and invigorating muscle has achieved good effect in clinical application.Traditional Chinese medicine can repair damaged intestinal mucosa through oral Chinese medicine,enema,acupuncture and other methods to treat UC.In this paper,the mechanism of the damaged intestinal mucosal barrier of UC in recent years was reviewed from four aspects of biological barrier,mechanical barrier,chemical barrier and immune barrier,and the mechanism of the treatment of UC by the theory of supplementing fire and invigorating soil and invigorating muscle was reviewed.This theory has made progress in the experimental and clinical studies on the treatment of UC through traditional Chinese medicine decoction,acupuncture,enema and other comprehensive therapies.This reflects the dialectical thought of TCM in the treatment of UC and the advantages of multi-approaches and multi-targets,and also provides reference for the basic and clinical research on the pathogenesis and treatment of UC in the future.
