中药萜类成分抗肝纤维化机制研究进展

摘　　要：肝纤维化(hepatic fibrosis,HF)是各种肝脏疾病导致肝细胞受伤后发生的再生过程,以细胞外基质(extracellular matrix,ECM)过度沉积形成大量瘢痕组织为主要病理特征。HF具有可逆性,若不及时干预,将会恶化为肝硬化,甚至进展为肝癌等疾病。因此,早期逆转HF是挽救千万肝病患者的有效手段。HF的发病机制复杂,目前临床尚无特效药物治愈HF。中药在防治HF方面具有较大潜力,中药有效成分抗HF的研究与应用日益增长。中药萜类成分通过抑制肝星状细胞(hepatic stellate cells,HSC)活化、ECM沉积、肝脏血管病理性生成以及促进HSC凋亡等途径改善HF。故该文以中药萜类成分为研究对象,对中药萜类成分涉及多信号通路、多靶点改善HF的作用机制进行归纳总结,以期为中药萜类成分抗HF的基础研究、临床应用以及新药研发提供思路和理论参考。Hepatic fibrosis(HF)is a regenerative process that occurs after hepatocyte injury caused by various liver diseases,which is characterized by excessive deposition of Extracellular matrix(ECM)to form a large number of scar tissues.HF is reversible,and without timely intervention,it will deteriorate into liver cirrhosis and even liver cancer.Therefore,early reversal of HF is an effective means to save millions of patients with liver disease.The pathogenesis of HF is complex,and there is no specific drug to cure HF.Traditional Chinese medicine has great potential in the prevention and treatment of HF,and the research and application of active components of traditional Chinese medicine in the treatment of HF are increasing.Traditional Chinese medicine terpenoids can improve HF by inhibiting the activation of Hepatic stellate cells(HSC),ECM deposition,hepatic vascular pathological formation and promoting HSC apoptosis.Therefore,this paper takes the terpenoids of traditional Chinese medicine as the research object,and summarizes the mechanism of action of terpenoids of traditional Chinese medicine in improving HF involving multiple signaling pathways and multiple targets,in order to provide ideas and theoretical reference for basic research,clinical application and new drug development of anti-HF.
