基于“肠-脑轴”理论探讨越鞠丸从心脾论治抑郁症

摘　　要：抑郁症是临床上常见的一种精神障碍类疾病,发病机制复杂。肠-脑轴是肠道微生物群与中枢神经系统之间的双向通信通路,在抑郁症的发生发展中发挥了重要作用。抑郁症归属于中医“郁证”范畴,思则气结、久伤心脾是郁证的关键病机。肠-脑轴的功能障碍与中医脾失健运、心失所养互为因果。有研究表明越鞠丸具有显著的抗抑郁作用,并且有利于调节肠道微生物群平衡,恢复肠道内环境稳态。该文拟从肠-脑轴的生理学基础出发,立足于中医“心脾相关”理论及现代研究,通过阐明脑肠与心脾两脏在功能上的统一性以及在抑郁症中的调节作用,对越鞠丸从心脾治疗抑郁症进行理论探析,以期为中医治疗抑郁症提供新思路。Depressive disorder is a common clinical mental disease with a complex pathogenesis.The gut-brain axis,a bidirectional communication pathway between the gut microbiota and the central nervous system,plays an important role in the development of depressive disorder.Depressive disorder belongs to the category of“depressive syndrome"in traditional Chinese medicine(TCM).The key of the depression is that contemplation causes Qi stagnation,which long-term damages the heart and the spleen.The dysfunction of the gut-brain axis and the spleen and heart deficiency in TCM are mutually beneficial.Studies have shown that Yueju Pill(越丸)has significant anti-depression effect,and is conducive to regulate the balance of intestinal microbiota and restore the intestinal homeostasis.Based on the physiological basis of gut-brain axis and“heart-spleen correlation"theory in TCM and modern studies,this paper aims to clarify the functional unity of the two organs and their regulating role in depressive disorder.To explore the theory of Yueju Pill in treating depressive disorder from the heart and spleen,in order to provide a new idea for the treatment of depressive disorder in TCM.
