温肾健脾法治疗腹泻型肠易激综合征研究进展

摘　　要：腹泻型肠易激综合征(diarrhea-predominant irritable bowel syndrome,IBS-D)是临床常见的功能性紊乱疾病,西医尚未明确阐明其发病机制,治疗上尚缺乏有效的治疗手段。中医药具有多靶点、多途径、多通路等优点,可发挥综合治疗作用,能有效改善IBS-D的临床疗效及症状,并可降低复发率。在中医辨证理论中,脾肾阳虚为IBS-D的重要病机,贯穿IBS-D病机演变的始终。温肾健脾法作为治疗IBS-D的经典治法,被广泛应用于中药复方治疗、针灸治疗、穴位贴敷治疗、中西医结合治疗、内外合治疗法及其他中医特色疗法治疗IBS-D当中,且疗效显著。结合目前相关研究,温肾健脾法发挥治疗作用的机制可能与降低内脏敏感、调节脑肠互动、调控机体免疫及减轻炎症反应有关。文章主要从温肾健脾法治疗IBS-D的临床应用及作用机制两方面概述其研究进展,以期为临床治疗IBS-D提供更好的方案及思路。Diarrhea-predominant irritable bowel syndrome(IBS-D)is a common clinical functional disorder.Western medicine has not yet clarified its pathogenesis,and there is still a lack of effective treatment.Traditional Chinese medicine has the advantages of multi-target,multi-channel and multichannel,which can play a comprehensive therapeutic role,effectively improve the clinical efficacy and symptoms of IBS-D,and reduce the recurrence rate.In TCM syndrome differentiation theory,spleen and kidney Yang deficiency is an important pathogenesis of IBS-D,throughout the evolution of IBS-D pathogenesis.As a classic treatment for IBS-D,warming kidney and invigorating spleen method is widely used in traditional Chinese medicine compound treatment,acupuncture treatment,acupoint application treatment,integrated traditional Chinese and Western medicine treatment,internal and external treatment and other traditional Chinese medicine characteristic therapy for IBS-D,and the curative effect is remarkable.Combined with the current research,the therapeutic mechanism of warming kidney and strengthening spleen may be related to reducing visceral sensitivity,regulating brain-gut interaction,regulating body immunity and reducing inflammatory response.This article summarizes the research progress in the clinical application and mechanism of warming kidney and strengthening spleen in the treatment of IBS-D,in order to provide better solutions and ideas for clinical treatment of IBS-D.
