芍药甘草汤加味联合中药熏洗治疗阴虚血瘀型糖尿病周围神经病变临床研究

摘　　要：目的探讨芍药甘草汤加味联合中药熏洗治疗阴虚血瘀型糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)患者临床疗效以及对血管内皮生长因子B(vascular endothelial growth factor B,VEGF-B)、内皮细胞特异分子1(endothelial cell specific molecule 1,ESM-1)的影响。方法108例患者随机分为中药联合组和对照组,每组54例,对照组给予常规治疗(甲钴胺片联合硫辛酸胶囊口服),中药联合组在对照组基础上加用芍药甘草汤加味联合中药熏洗治疗,疗程3个月。检测临床疗效、中医证候积分、多伦多临床神经病变评分(toronto clinical score system,TCSS)、运动神经传导速度(motor nerve conduction velocity,MCV)、感觉神经传导速度(sensory nerve conduction velocity,SCV)、VEGF-B、ESM-1、不良反应发生率变化。结果中药联合组临床有效率高于对照组(P<0.05),不良反应发生率低于对照组(P<0.05)。治疗后,中药联合组中医证候积分、TCSS评分、VEGF-B、ESM-1低于对照组(P<0.05),MCV、SCV高于对照组(P<0.05)。结论芍药甘草汤加味联合中药熏洗治疗阴虚血瘀型DPN患者临床效果显著,能够有效改善临床症状,提高神经传导功能,抑制血清VEGF-B、ESM-1水平,临床安全性良好,值得临床进一步推广应用。Objective To explore the clinical effects of Modified Shaoyao Gancao Decoction(药甘草汤加味)combined with herbal fumigation on diahetic peripheral neuropathy(DPN)with Yin deficiency and blood stasis syndrome and effect on vascular endothelial growth factor B(VEGF-B)and endothelial cell specific molecule 1(ESM-1).Methods 108 patients were randomly assigned into control group(54 cases)for 2-week intervention of conventional treatment(MeCobalamin Tablets combined with Thioctic Acid Capsules oral),and TCM combined group(54 cases)for 2-week intervention of Modified Shaoyao Gancao Decoction combined with herhal fumigation and conventional treatment.The course of treatment is 3 months.The changes in clinical efficacy,TCM symptom scores,Toronto clinical scoring system scores,motor nerve conduction velocity(MCV),sensory nerve conduction velocity(SCV),VEGF-B,ESM-1 and incidence of adverse reactions were detected.Results The TCM combined group demonstrated higher clinical effective rate rate than the control group(P<0.05),together with lower incidence of adverse reactions than the control group(P<0.05).After the treatment,the TCM combined group displayed lower TCM symptom scores,TCSS scores,VECF-B and ESM-1 than the control group(P<0.05),along with higher MCV and SCV than the control group(P<0.05).Conclusion Modified Shaoyao Gancao Decoction combined with herbal fumigation on DPN with yin deficiency and blood stasis syndrome has a very significant clinic effect.It can effectively improve the clinical symptoms and the nerve conduction function,and inhibit the levels of serum VEGF-B and ESM-1.It has good clinical safety and is worthy of further promotion and application in the clinic.
