自拟二黄二冬汤治疗食管癌患者放射性肺炎的临床研究

摘　　要：目的:观察自拟二黄二冬汤治疗食管癌放射治疗导致放射性肺炎的临床疗效及对患者肺功能、炎症因子的影响。方法:收集2018年12月-2020年12月江苏省中医院放疗科收治的根治性同步放化疗后发生放射性肺炎的食管癌患者65例,分为对照组35例、中药治疗组30例。对照组给予必要的泼尼松和(或)抗生素治疗,中药治疗组加用自拟二黄二冬汤,比较两组患者临床疗效,以及患者肺功能及血清细胞炎症因子变化。结果:中药治疗组在中医症状和肺功能改善方面优于对照组(P<0.05);中药治疗组对气阴两虚、痰热蕴肺证候的治疗有效率高于对照组(P<0.05),同时中药治疗组放射性肺炎的治愈时间显著短于对照组(P<0.05);中药治疗组患者体内血清中炎症因子浓度显著降低(P<0.05)。结论:采用自拟二黄二冬汤治疗食管癌患者放射性肺炎的临床疗效显著,可明显改善肺功能和缩短痊愈时间,其机制可能与降低血清炎症因子浓度有关。Objectire:To observe the effect of the Erhuang Erdong decoction(二黄二冬汤)on the clinical efficacy,lung function,and inflammatory factors in patients with radiation pneumonia caused by radiotherapy for esophageal cancer.Methods:A total of 65 patients with esophageal cancer who developed radiation pneumonia after radical concurrent radiotherapy and chemotherapyO were collected from Dec.2018 to Dec.2020,and 35 cases in which were set as the control group while another 30 cases were in the TCM group.The control group was given the necessary prednisone and/or antibiotic treatment,and the TCM group was given the Erhuang Erdong decoction.The clinical efficacy and the changes of serum cytoinflammatory factors were compared between the two groups.Results:The TCM group was better than the control group in terms of TCM symptoms and lung function improvement(P<0.05),the TCM group was more effective in treating Qi(气)and Yin(阴)deficiency and phlegm-heat accumulation in the lung syndrome than the control group(P<0.05),and the cure time of acute radiation pneumonia in the TCM group was significantly shorter than that in the control group(P<0.05).The concentration of inflammatory factors in the serum of patients in the TCM group was significantly reduced(P<0.05).Conclusion:The Erhuang Erdong decoction is effective on radiation pneumonitis in patients with esophageal cancer,can significantly improve lung function,and shorten the recovery time.The mechanism may be related to reducing the concentration of serum inflammatory factors.
