“标本根结”理论指导下针灸远近配穴的临床应用理论基础

摘　　要：远近配穴作为针灸治疗腧穴配穴的主要方法之一,临床已经得到广泛应用。中医学目前尚未对远近配穴概念及其理论进行定义,导致中医医生和针灸医生只能从文字字面意思出发,自行领会及理解远近配穴的使用,目前较为认可的方法为以疾病病变部位为中心,将其视为近端,其远端穴位则以病变部位远端为标准,该方法简单易行,但对远近配穴的深层定义存在明显的局限性,不利于临床实践开展及应用。鉴于目前尚未有明确教材和文献对其进行阐述,认为远近配穴为穴位之关系,亦为标本兼治理论的体现,可指导临床疾病的辨证施治、选穴取穴。标本根结理论指导下选穴配穴十分灵活,根(本)、结(标)治疗可取近、取远,亦可远近结合,标本根结同治。在实践应用过程中做好远近配穴标本根结同治,更好的畅通经络原气、沟通气血,补充了十二经脉阴阳相贯、心脉循环的理论,扩大了腧穴主治之范围,从而更好地为临床治疗提供指导。As one of the main methods of acupuncture and moxibustion for acupoint matching,far and near matching acupoint has been widely used in clinic.At present,the concept and theory of far-near matching acupoints are still not defined in TCM,which leads to TCM doctors and acupuncturists cao only understand the use of far-near matching acupoints from the literal meaning ofA words.At present,the more recognized method is to take the diseased part as the center and regard it as the proximal part,while the remote acupoint is based on the distal part of the diseased part.The method is simple and easy to apply,but the deep definition of far-near matching acupoints has obvious limitations,which is not conducive to clinical practice and application.In view of the fact that there is no clear teaching material and literature to elaborate it at present,the relationship between far and near acupoints is also the embodiment of the theory of treating both the symptoms and the root causes,which can guide the differential treatment of clinical diseases and the selection of acupoints.Under the guidance of the origin and terminal theory,the selection of acupoints and the matching acupoints are very flexible.Acupoints can be taken from near to far,or they can be combined,and the origin and terminal can be treated together.In the process of practice and application,we should do a good job in treating origin and terminal,in order to better unblock the primary Qi(气)of meridians,communicate Qi and blood,supplement the twelve meridians’s theory of Yin-Yang(阴阳)intersection and circulation of heart and pulse,and expand the scope of acupoint indications,so as to provide a better guidance for clinical treatment.
