马鸿斌教授运用行湿补气养血汤治疗慢性肾功能衰竭的经验

摘　　要：慢性肾功能衰竭是指各种原发性或继发性慢性肾脏疾病导致的肾功能损害所引起的临床综合征,主要临床表现有疲乏、纳差、恶心、呕吐等,其病机复杂,发病率也在逐渐升高。目前,西医缺乏特效药物及有效的治疗方法,且治疗疗程较长、费用较高。近年来,越来越多的研究证明了中医治疗该病可有效延缓疾病的发展,改善临床症状,提高患者的生活生存质量。马鸿斌教授专注于中医治疗慢性肾脏疾病30余载,认为慢性肾功能衰竭的病因病机以脾肾两虚、湿浊毒蕴最为常见,故常以“脾肾同治、标本兼顾”的行湿补气养血汤为基础方,再根据患者的个体情况辨证施治,临床疗效显著。笔者有幸跟随马教授临床学习,深刻感受到马教授用方遣药之精准以及此方对慢性肾功能衰竭之气虚湿阻证的临床治疗效果,故本文从中医病因病机、行湿补气养血汤的方义解析及其辨证化裁、特色用药、典型医案举例四个方面将马鸿斌教授运用行湿补气养血汤治疗慢性肾功能衰竭的经验总结如下,以望为临床用方用药提供新思路,为中医药治疗慢性肾功能衰竭疾病提供理论依据及临床经验。Chronic renal failure refers to the clinical syndrome due to renal dysfunction caused by various primary or secondary chronic kidney diseases.The main clinical manifestations are fatigue,anorexia,nausea,vomiting,etc..It pathogenesis is complex,and the incidence rate is gradually increasing.At present,there are no specific medicine and specific effective treatment forC treating the disease in Western medicine,the course of conventional treatment method is long,and the cost is high.In recent years,more and more studies have demonstrated that TCM treatment can effectively delay the progression of the disease,ameliorate the clinical symptoms of patients,and improve their quality of life.Professor MA Hongbin has beem focusing on the treatment of chronic kidney diseases in TCM for more than 30 years.The professor believes that the etiology and pathogenesis of chronic kidney failure are most both spleen and kidney deficiency,and dampness and toxicity accumulation.Therefore,the professor often uses the Xingshi Buqi Yangxue decoction(行湿补气养血汤)for dispelling dampness,replenish Qi(气)and nourishing blood to achieve the purpose of treating the spleen and kidney simultaneously and taking care of both the symptoms and the root cause based on syndrome differentiation.The clinical effect is remarkable.The author had the honor to learn from Professor MA,and deeply felt the accuracy of Professor MA’s prescription and the clinical effect of this prescription on chronic renal failure with Qi deficiency and dampness stagnation syndrome.Professor MA Hongbin’s experience in treating chronic renal failure with the Xingshi Buqi Yangxue decoction is summarized in this paper from four aspects of the etiology and pathogenesis in TCM of the disease,the prescription analysis and its differentiation,the characteristic medication,and typical medical cases,hoping to provide new ideas for the clinical application of prescriptions and medicines,and provide a theoretical basis and clinical experience for treating chronic renal fai
