中西医结合治疗外阴白斑疗效的Meta分析

摘　　要：目的:评价中西医结合治疗外阴白斑的干预效果。方法:计算机检索万方、中国知网、维普等数据库中有关中西医结合治疗外阴白斑的随机对照试验,检索时限从建库至2022年6月。由两名经过培训的研究者独立筛检文献、提取数据并对符合纳入和排除标准的文献进行质量评价。使用RevMan5.4进行Meta分析。结果:共纳入文献16篇,涉及患者1715例。Meta分析结果显示:中西医结合治疗后,试验组中医证候积分低于对照组(SMD=-1.79,95%CI为[-2.55,-1.02],P<0.05),CD3^(+)细胞分数低于对照组(SMD=-1.75,95%CI为[-2.06,-1.44],P<0.05),CD4^(+)/CD8^(+)细胞比值低于对照组(SMD=-1.16,95%CI为[-1.87,-0.44],P<0.05),临床治疗效果优于对照组(SMD=4.39,95%CI为[3.27,5.87],P<0.05),疾病复发率低于对照组(SMD=0.28,95%CI为[0.15,0.52],P<0.05)。结论:现有证据提示,中西医结合疗法能有效改善外阴白斑患者的临床症状,提升临床治疗效果,降低复发率。因此,建议将中医疗法纳入到外阴白斑治疗方案中,并期待后续开展更多高质量的多中心试验。Objective:To evaluate the intervention effect of the integrative medicine in the treatment of vulva leukoplakia.Methods:Randomized controlled trials(RCTs)concerning the treatment of vulvae leukoplakia in the integrative medicine were retrieved by computer from Wanfang,CNKI,VIP and other databases.Articles were independently screened,and data extracted and quality evaluated for articles that met the inclusion criteria were performed by two trained investigators.A Meta-analysis was performedO using RevMan 5.4.Results:A total of 16 articles involving 1715 patients were included.The results of the Meta-analysis showed that the test group had lower score of TCM syndrome than the control group after the integrative medicine treatment(SMD=-1.79,95%CI[-2.55,-1.02],P<0.05),CD3^(+)cell scores were lower than those in the control group(SMD=-1.75,95%CI[-2.06,-1.44],P<0.05),CD4^(+)/CD8^(+)cell ratio was lower than that in the control group(SMD=-1.16,95%CI[-1.87,-0.44],P<0.05),the clinical treatment effect in the experimental group was superior to that in the control group(SMD=4.39,95%CI[3.27,5.87],P<0.05),and the disease recurrence rate in the experimental group was lower than the control group(SMD=0.28,95%CI[0.15,0.52],P<0.05).Conclusion:The existing evidence suggests that the integrative medicine is an effective treatment for vulvar leukoplakia,and that it can effectively improve the clinical symptoms of patients with vulvar leukoplakia,enhance the clinical therapeutic effect,and reduce the recurrence rate.Therefore,it is recommended to incorporate TCM therapy into the treatment plan for vulvar leukoplakia,and more high-quality multi-center trials are expected to be conducted in the future.
