摘  要:目的 探讨3D打印技术下个体化反折包埋式髋臼假体在治疗成人Crowe Ⅳ发育性髋关节发育不良(developmental dysplasia of the hip, DDH)患者的应用。方法 对20例成人Crowe Ⅳ DDH患者根据三维CT数据,制作骨盆3D打印模型,在模型上模拟全髋关节置换时的髋臼,采用藻酸盐印模材料修复髋臼缺损,定制个体化髋臼假体,术中采用手术方案与术前模拟操作一致。记录髋臼假体型号、手术时间、术中出血量、术后Harris评分、VAS评分及术后并发症的发生率。结果 术中使用髋臼假体型号与定制髋臼假体一致,3D打印组手术时间、术中出血量、术后Harris评分、VAS评分、术后并发症发生率均优于常规组。结论 3D打印技术下个体化反折包埋式髋臼假体在成人CroweⅣDDH患者进行THA手术时修复髋臼缺损优势明显,增加髋臼假体稳定性,减少手术时间、术中出血及术后并发症,明显改善髋关节功能,提高了手术效率和准确性,值得临床应用与推广。Objective To investigate the application of individualized refracted embedded acetabular prosthesis with 3D printing technology in the treatment of adult Crowe IV DDH patients.Methods According to 3D CT data,20 adult Crowe IV DDH patients were used to make a 3D printed model of the pelvis.The model was used to simulate the acetabulum during total hip replacement.Alginate impression materials were used to repair the acetabulum defect,and acetabulum prosthesis was customized.The operation plan adopted during the operation was consistent with that before the operation.The type of acetabular prosthesis,operation time,intraoperative blood loss,postoperative Harris score,VAS score and the incidence of postoperative complications were recorded.Results The type of acetabular prosthesis used in the operation was consistent with the customized acetabular prosthesis.The operation time,intraoperative blood loss,Harris score,VAS score and postoperative complication rate in the 3D printing group were better than those in the conventional group.Conclusion The application of individualized reverse folding embedded acetabular prosthesis under 3D printing technology in adult Crowe IV DDH patients has obvious advantages in repairing acetabular defects during THA,increasing the stability of acetabular prosthesis,reducing operation time,intraoperative bleeding and postoperative complications,significantly improving hip joint function as well as improving the efficiency and accuracy of surgery,which is worthy of clinical application and promotion.
