三维双回波稳态序列在胫骨平台骨折患者半月板损伤诊断中的应用价值

摘  要:目的:探讨与分析核磁共振成像三维双回波稳态序列(3D-DESS)在胫骨平台骨折患者半月板损伤诊断中的应用价值,希望早期检出半月板损伤。方法:选择我院2021年1月-2023年1月进行诊治的胫骨平台骨折患者88例作为研究对象,所有患者都给予核磁共振成像检查,记录影像学成像特征。以关节镜诊断作为金标准,判断三维双回波稳态序列诊断半月板损伤的敏感性与特异性。结果:在88例患者中,关节镜检查判断为半月板损伤40例(半月板损伤组),占比45.5%。半月板损伤组的膝关节Lysholom评分与非半月板损伤组对比无明显差异(P>0.05)。半月板损伤组的骨隧道扩大率、三维双回波稳态序列T2值都显著高于非半月板损伤患者(P<0.05)。在88例患者中,三维双回波稳态序列判断为半月板损伤39例,三维双回波稳态序列在胫骨平台骨折患者半月板损伤诊断中的敏感性与特异性分别为97.5%(39/40)和100.0%(48/48)。结论:胫骨平台骨折患者发生半月板损伤比较常见,核磁共振成像双回波稳态序列可有效反映半月板的生理与病理状况,对于诊断半月板损伤具有很高的敏感性与特异性,值得在临床上推广应用。Objective:To investigate and analyze the application value of three-dimensional double-echo steady-state sequence(3D-DESS)of magnetic resonance imaging in the diagnosis of meniscus injury in patients with tibial plateau fracture,hoping to detect meniscus injury early.Methods:88 patients with tibial plateau fracture diagnosed and treated in our hospital from January 2021 to Janu-ary 2023 were selected as research objects.All patients were given MRI and imaging characteristics were recorded.All patients were also given arthroscopy,and arthroscopy was used as the gold standard to determine the sensitivity and specificity of three-dimensional double-echo steady-state sequence in the diagnosis of meniscus injury.Results:Among the 88 patients,40 cases(meniscus injury group)were diagnosed by arthroscopy,accounting for 45.5%.There were no significant diferences in systolic blood pressure,gender,age,body mass index,heart rate and diastolic blod pressure in meniscus injury group compared with non-meniscus injury group(P>0.05).There was no significant difference in Lysholom score between the meniscus injury group and the non-meniscus injury group(P>0.05).The expansion rate of bone tunnel and T2 value of three-dimensional double-echo steady-state sequence in meniscus injury group were significantly higher than those in non-meniscus injury patients(P<0.05).Among the 88 patients,39 cases were judged as meniscus injury by three-dimensional double-echo steady-state sequence.The sensitivity and specificity of three-dimensional doub-le-echo steady-state sequence in the diagnosis of meniscus injury in patients with tibial plateau fracture were 97.5%(39/40)and 100.0%(48/48).Conclusion:Meniscus injury is common in patients with tibial plateau fracture.Mri double-echo steady-state se-quence can effectively reflect the physiological and pathological status of meniscus,which has high sensitivity and specificity for the diag-nosis of meniscus injury,and is worthy of clinical application.
