临床药师参与1例海洋创伤弧菌致脓毒症病例的药学实践

摘　　要：1例70岁女性患者因“虾刺伤后2 d,意识不清2 h”入院,患者病情进展迅速,并出现多器官功能障碍表现,伤口组织物宏基因组学提示创伤弧菌,确诊为创伤弧菌感染导致的脓毒症。临床药师对该患者进行全程药学监护,在抗感染、围手术期抗凝方案的制定与调整方面,提出用药建议,发挥了积极作用。经治疗,患者感染得到有效控制,INR控制在目标范围内,痊愈出院。A 70-year-old female patient was admitted due to unconsciousness for 2 hours,2 days after shrimp injury.The patient's condition progressed rapidly and showed multiple organ dysfunction.And she was diagnosed with sepsis caused by Vibrio vulnificus infection according to the results of wound tissue metagenomics.The clinical pharmacist provided full pharmaceutical monitoring to her and actively put forward medication suggestions in the formulation and adjustment of anti-infection and perioperative anticoagulation plans.After treatment,the patient's infection was effectively controlled,the INR was within the target range.She was cured and discharged.
