血液免疫项目ELISA法检测中采用EDTA-K_(2)抗凝剂、枸橼酸-枸橼酸钠抗凝剂的作用研究

摘　　要：目的探讨乙二胺四乙酸二钾(EDTA-K_(2))抗凝剂、枸橼酸-枸橼酸钠抗凝剂对血液免疫项目酶联免疫吸附试验(ELISA)法检测结果的影响。方法48689份无偿献血者样本,所有样本分别采用EDTA-K_(2)抗凝剂抗凝、枸橼酸-枸橼酸钠抗凝剂抗凝。将抗凝标本采用ELISA法检测抗-人免疫缺陷病毒(HIV)、抗-丙型肝炎病毒(HCV)、乙型肝炎病毒表面抗原(HBsAg)及抗-梅毒螺旋体(TP),统计两种抗凝血液样本的阳性标本数量。将EDTA-K_(2)抗凝剂改用枸橼酸-枸橼酸钠抗凝剂,将阳性结果的血样进行乙型肝炎病毒(HBV)聚合酶链式反应(PCR)扩增复检。统计辫血复检结果。结果新创厂家生产的EDTA-K_(2)抗凝检出抗-HIV阳性71例(0.15%),抗-HCV阳性628例(1.29%),HBsAg阳性784例(1.61%),抗-TP阳性72例(0.15%);枸橼酸-枸橼酸钠抗凝检出抗-HIV阳性57例(0.12%),抗-HCV阳性414例(0.85%),HBsAg阳性652例(1.34%),抗-TP阳性44例(0.09%)。万泰厂家生产的EDTA-K_(2)抗凝检出抗-HIV阳性65例(0.13%),抗-HCV阳性511例(1.05%),HBsAg阳性721例(1.48%),抗-TP阳性72例(0.15%);枸橼酸-枸橼酸钠抗凝检出抗-HIV阳性56例(0.12%),抗-HCV阳性467例(0.96%),HBsAg阳性623例(1.28%),抗-TP阳性47例(0.10%)。将EDTA-K_(2)抗凝剂改用枸橼酸-枸橼酸钠抗凝剂后,对所有阳性样本采取HBsAg做PCR复检阳性率,新创、万泰阳性检出率分别降到了1.35%(657/48689)和1.33%(647/48689)。结论EDTA-K_(2)抗凝剂对ELISA法检测结果的影响较枸橼酸-枸橼酸钠抗凝剂的干扰更明显。Objective To discuss the effect of dipotassium ethylenediaminetetraacetic acid(EDTA-K_(2))anticoagulant and citric acid-sodium citrate anticoagulant in enzyme linked immunosorbent assay(ELISA)detection of blood immunizationg items.Methods 48689 samples of unpaid blood donors were collected,and all samples were anticoagulated with EDTA-K_(2)anticoagulant and citric acid-sodium citrate anticoagulant respectively.The anticoagulant samples were detected by ELISA for anti-human immunodeficiency virus(HIV),anti-hepatitis C virus(HCV),hepatitis B virus surface antigen(HBsAg)and Treponema pallidum(TP),and the number of positive samples of the two anticoagulant blood samples was counted.The EDTA-K_(2)anticoagulant was changed to citric acid-sodium citrate anticoagulant,and the blood samples with positive results were retested for hepatitis B virus(HBV)by polymerase chain reaction(PCR)amplification.The retest results was counted.Results EDTA-K_(2)anticoagulation from Xinchuang manufacturer detected anti-HIV positivity in 71 cases(0.15%),anti-HCV positivity in 628 cases(1.29%),HBsAg positivity in 784 cases(1.61%),and anti-TP positivity in 72 cases(0.15%);citric acid-sodium citrate anticoagulant detected anti-HIV positivity in 57 cases(0.12%),anti-HCV positivity in 414 cases(0.85%),HBsAg positivity in 652 cases(1.34%)and anti-TP positivity in 44 cases(0.09%).EDTA-K_(2)anticoagulant from Wantai manufacturer detected anti-HIV positivity in 65 cases(0.13%),anti-HCV positivity in 511 cases(1.05%),HBsAg positivity in 721 cases(1.48%),and anti-TP positivity in 72 cases(0.15%);acid-sodium citrate anticoagulant detected anti HIV-positivity in 56 cases(0.12%),anti-HCV positivity in 467 cases(0.96%),HBsAg positivity in 623 cases(1.28%),and anti-TP positivity in 47 cases(0.10%).After EDTA-K_(2)anticoagulant was changed to citric acid-sodium citrate anticoagulant,the positive rate of HBsAg retest by PCR for all positive samples,and the detection rates of Xinchuang and Wantai were reduced to 1.35%(657/48689)and 1.33%(647/48689),respectiv
