阴道内置与舌下含服米索前列醇在门诊无痛人流手术中的应用效果评价

摘　　要：目的观察阴道内置与舌下含服米索前列醇在门诊无痛人流手术中的应用效果。方法80例门诊无痛人流手术女性患者,采用抽签法分为对照组和实验组,每组40例。两组患者均实施无痛人流手术,对照组患者术前阴道内置米索前列醇,实验组患者术前舌下含服米索前列醇。比较两组患者临床治疗效果、手术相关指标、不良反应发生情况。结果实验组患者的总有效率为97.50%(39/40),明显高于对照组的80.00%(32/40),差异具有统计学意义(P<0.05)。实验组患者的手术时间(5.02±0.35)min短于对照组的(7.01±1.47)min,术中出血量(16.08±2.74)ml少于对照组的(21.67±8.09)ml,差异具有统计学意义(P<0.05)。两组患者胃肠道反应发生率比较差异无统计学意义(P>0.05);实验组患者的下腹痛、术中阴道出血发生率分别为0、2.50%,均低于对照组的12.50%、15.00%,差异具有统计学意义(P<0.05)。结论相较于阴道内置米索前列醇的给药形式,门诊行无痛人流手术采用的舌下含服方式具有更高的疗效,其具有用药方便、吸收效果好等优势,可有效减少阴道内置给药的繁琐性和对患者机体造成的创伤性,其可被作为术前给药的第一选择。但若患者的妊娠反应较重、舌下含服困难,阴道内置给药可为最佳方式。基于此,临床患者的实际表现情况为选择给药方式的重要性依据。Objective To observe the application effect of intravaginal and sublingual administration of misoprostol in outpatient painless abortion.Methods A total of 80 female outpatients with painless abortion were divided into control group and experimental group by lottery method,with 40 cases in each group.Painless abortion was performed in both groups.The control group had intravaginal administration of misoprostol before surgery,and the experimental group had sublingual administration of misoprostol before surgery.The clinical therapeutic effect,surgery-related indexes and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the experimental group was 97.50%(39/40),which was significantly higher than that of 80.00%(32/40)of the control group,and the difference was statistically significant(P<0.05).The operative time of(5.02±0.35)min in the experimental group was shorter than that of(7.01±1.47)min in the control group,and the intraoperative blood loss of(16.08±2.74)ml was less than that of(21.67±8.09)ml in the control group.The differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of gastrointestinal reactions between the two groups(P>0.05).The incidence rates of lower abdominal pain and intraoperative vaginal bleeding in the experimental group were 0 and 2.50%,which were lower than those of 12.50%and 15.00%in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with intravaginal administration of misoprostol,the sublingual administration method used in the outpatient painless abortion has higher efficacy,and has the advantages of convenient administration and good absorption effect,which can effectively reduce the complexity of intravaginal administration and the trauma caused to the patient's body,and can be used as the first choice for preoperative administration.However,if the patient's pregnancy reaction is severe and it is difficult to take sublin
