复方丙酸氯倍他索软膏与糠酸莫米松乳膏治疗手部湿疹患者的效果对照分析

摘　　要：目的对比复方丙酸氯倍他索软膏、糠酸莫米松乳膏治疗手部湿疹的效果。方法78例手部湿疹患者,据治疗方式不同分为对照组和研究组,每组39例。对照组行糠酸莫米松乳膏治疗,研究组行复方丙酸氯倍他索软膏治疗。对比两组患者治疗前后湿疹面积及严重程度指数量表(EASI)评分、治疗效果、不良反应发生情况。结果治疗前,两组患者皮损面积、瘙痒程度、苔藓样变评分对比差异无统计学意义(P>0.05);治疗后,两组患者皮损面积、瘙痒程度、苔藓样变评分均较治疗前降低,研究组患者皮损面积、瘙痒程度、苔藓样变评分分别为(1.35±0.41)、(0.96±0.22)、(0.96±0.33)分,均显著低于对照组的(2.69±0.35)、(1.87±0.43)、(1.67±0.48)分,差异具有统计学意义(P<0.05)。研究组患者治疗总有效率97.44%(38/39)显著高于对照组的71.79%(28/39),差异具有统计学意义(χ^(2)=9.8485,P=0.0017<0.05)。研究组患者的不良反应发生率12.82%(5/39)显著高于对照组的0,差异具有统计学意义(χ^(2)=5.3425,P=0.0208<0.05)。结论相较于糠酸莫米松乳膏,复方丙酸氯倍他索软膏救治手部湿疹的疗效和疾病症状改善效果更佳,但不良反应发生率更高,临床需结合患者具体情况选择用药。Objective To compare the effect of compound clobetasol propionate ointment and mometasone furoate cream in patients with hand eczema.Methods A total of 78 patients with hand eczema were divided into control group and study group according to different treatment methods,with 39 cases in each group.The control group was treated with mometasone furoate cream,and the study group was treated with compound clobetasol propionate ointment.The score of eczema area and severity index(EASI)before and after treatment,therapeutic effect,and occurrence of adverse reactions were compared between the two groups.Results Before treatment,there was no statistically significant difference in scores of lesion area,pruritus degree and lichenoid lesion between the two groups(P>0.05).After treatment,the scores of lesion area,pruritus degree and lichenoid lesion in both groups were lower than those before treatment in this group;the scores of lesion area,pruritus degree and lichenoid lesion in the study group were(1.35±0.41),(0.96±0.22)and(0.96±0.33)points,which were significantly lower than those of(2.69±0.35),(1.87±0.43)and(1.67±0.48)points in the control group;the differences were statistically significant(P<0.05).The total effective rate of the study group was 97.44%(38/39),which was higher than that of 71.79%(28/39)of the control group,and the difference was statistically significant(χ^(2)=9.8485,P=0.0017<0.05).The incidence of adverse reactions in the study group was 12.82%(5/39),which was higher than that of 0 in the control group,and the difference was statistically significant(χ^(2)=5.3425,P=0.0208<0.05).Conclusion Compared with mometasone furoate cream,compound clobetasol propionate ointment has better curative effect and improvement effect of disease symptoms in the treatment of hand eczema,but the incidence of adverse reactions is higher.Clinically,medication should be selected according to the specific situation of patients.
