毛发移植术中两种受损单株生长期毛囊单位移植效果观察

摘　　要：目的毛囊损伤是毛发移植术中常见的现象,本研究拟对两种常见术中损伤的单株生长期毛囊进行移植并观察1年后的生长效果,为减少毛囊资源浪费,提高利用率,增加移植密度,提高手术效果提供参考。方法选取本院整形科2017年10月至2020年1月的52例前额发际线缺陷的手术患者作为研究对象,其中男24例,女28例。移植单位数:1,450~2,827株。选取正常、毛乳头缺失和外毛根鞘下1/2缺损失的生长期单株毛囊单位作为研究对象,分别种植在3个固定区域,观察1年后的生长效果。结果正常单株毛囊单位存活率为(88.46±5.26)%;毛乳头缺失的毛囊单位存活率为(65.13±9.07)%,生长方向较自然,毛发光泽较好;外毛根鞘缺失下1/2的毛囊存活率(17.37±4.568)%,有细发或生长方向不自然的情况,与皮肤呈角度偏大,有部分贴紧头皮,三者之间差异存在统计学意义(P<0.05)。结论外毛根鞘缺失下1/2的毛囊单位的存活率低于毛乳头缺失的毛囊单位,且两者的存活率均低于正常毛囊单位。两种受损的毛囊可以种植在发际线以上的隐蔽位置或与正常的毛囊同时种植。同时,医护人员在手术中应提高分离精度,减少因提取和切割不当造成毛囊单位的损伤,减少损失,提高利用率。Objective Hair follicle damage is a common phenomenon in hair transplantation.In this study,we propose to transplant single growth phase hair follicles with two common intraoperative damages and observe the growth results after one year,in order to provide reference for reducing the waste of hair follicle resources,improving the utilization rate,increasing the transplant density and improving the surgical results.Methods Fifty-two surgical patients with forehead hairline defects from October 2017 to January 2020 in the plastic surgery department in our hospital were selected for the study,of whom 24 were male and 28 were female.Number of transplant units:1,450 to 2,827.Single hair follicular units with normal,missing hair papillae and loss of the lower 1/2 of the outer hair root sheath in the growth phase were selected as study subjects and planted in three fixed areas to observe the growth effect after one year.Results The survival rate of normal single follicular units was(88.46±5.26)%;the survival rate of follicular units with missing hair papilla was(65.13±9.068)%,with more natural growth direction and better hair lustre;the survival rate of follicles with missing lower 1/2 of the outer hair root sheath was(17.37±4.57)%,with fine hair or unnatural growth direction,with an angle to the skin,partially close to the scalp,and the difference between the three was statistically significant(P<0.05).Conclusion The survival rate of follicular units in the lower 1/2 of the missing outer hair sheath is lower than that of follicular units in the missing hair papilla,and both are lower than that of normal follicular units.They can be planted in hidden locations above the hairline or at the same time as normal hair follicles.The surgeon should also improve the precision of separation during the procedure to reduce damage to the follicular units due to improper extraction and cutting,to reduce loss and improve utilisation.
