摘　　要：目的评价椎弓根钉棒系统内固定治疗骨盆前环不稳定性骨折的临床疗效。方法回顾性分析2019年6月至2022年6月浙江省舟山医院收治的50例骨盆前环不稳定骨折患者的临床资料,其中使用椎弓根钉棒系统内固定(观察组)和外固定支架固定(对照组)治疗的患者各25列,比较两组患者手术时间、术中出血量、下床活动时间、负重时间、术后2周视觉模拟评分法(VAS)评分、术后并发症、Matta骨折复位情况和Majeed功能评分。结果两组患者在手术时间、术中出血量、术后并发症等方面比较差异均无统计学意义(均P>0.05)。观察组患者下床活动时间、负重时间、术后2周VAS评分均优于对照组,差异均有统计学意义(均P<0.05);术后6个月Matta骨折复位情况和及术后1、3、6个月Majeed功能评分均优于对照组,差异均有统计学意义(均P<0.05)。结论与外固定支架相比,椎弓根钉棒系统内固定治疗骨盆前环不稳定性骨折,具有固定牢靠、复位佳、术后功能恢复佳等优点,是更适宜的治疗方法。Objective To evaluate the clinical effect of pedicle screw rod system internal fixation in the treatment of unstable pelvic anterior ring fractures.Methods The clinical data of 50 patients with unstable fractures of the anterior pelvic ring admitted to Zhejiang Zhoushan Hospital from June 2019 to June 2022 were retrospectively analyzed,including 25 cases treated with pedicle screw system(study group)and 25 cases treated with external fixation stent(control group).Operative time,intraoperative blood loss,ambulation time,weight-bearing time,VAS pain score 2 weeks after surgery,postoperative complications,Matta fracture reduction situation and Majeed function scores were analyzed and compared between the two groups.Results There were no significant differences in age,gender,operation time and intraoperative blood loss between the two groups(all P>0.05).The ambulation time and the weight-bearing time were shorter,the VAS scores 2 weeks after operation were lower in study group than those in control group(all P<0.05).The excellent and good rate of Matta fracture reduction situation 6 months after surgery and Majeed function scores 1 month,3 months and 6 months after surgery in the study group were better than those in the control group(all P<0.05).Conclusion Compared with external fixation stent,internal fixation with pedicle screw rod system has advantages of solid fixation,good reduction,and better postoperative functional recovery in the treatment of unstable pelvic anterior ring fractures,which provides a suitable treatment option for trauma doctors.
