摘 要:目的:分析宏基因组二代测序技术(mNGS)在神经外科颅内感染病原学诊断中的应用价值。方法:选择2020年1月—2022年12月收治的20例神经外科术后拟诊颅内感染患者为研究对象,两周后由3位神经外科专家共同判断是否确诊为颅内感染。对20例患者的脑脊液细菌培养结果和宏基因组二代测序检测结果进行对比。结果:宏基因组二代测序诊断神经外科术后颅内感染的灵敏度、特异度及准确率分别为100.0%(13/13),85.7%(6/7)及95.0%(19/20);细菌培养的灵敏度、特异度及准确率分别为69.2%(9/13),57.1%(4/7),65.0%(13/20);宏基因组二代测序的灵敏度、特异度及准确率均高于脑脊液细菌培养,差异有统计学意义(P<0.05)。结论:宏基因组二代测序技术在神经外科颅内感染病原学诊断中具有一定价值。Objective:To analyze the application of the second generation macro-genome sequencing technology(mNGS)in the etiology diagnosis of intracranial infection in neurosurgery.Methods:All 20 patients with intracranial infection after neurosurgery were selected from January 2020 to December 2022.After two weeks,three neurosurgery experts jointly judged whether the diagnosis was intracranial infection.The results of second generation of macrogenome sequencing technology in 20 patients,were compared with cerebrospinal fluid bacterial culture.Results:The sensitivity,specificity and accuracy of mNGS in the diagnosis of intracranial infection after neurosurgery were 100.0%(13/13),85.7%(6/7)and 95.0%(19/20),respectively.The sensitivity,specificity and accuracy of bacterial culture group were 69.2%(9/13),57.1%(4/7)and 65.0%(13/20)respectively.The sensitivity,specificity and accuracy of the second generation of macrogenome sequencing technology were higher than those of the conventional cerebrospinal fluid bacterial culture group,and the difference was statistically significant(P<0.05).Conclusion:The second generation sequencing technology of macrogenomes has certain value in the etiology diagnosis of intracranial infection in neurosurgery.
