保留皮桥部分缝合联合负压封闭引流术治疗骶尾部藏毛窦的效果分析

摘　　要：目的:分析保留皮桥部分缝合联合负压封闭引流术治疗骶尾部藏毛窦的效果。方法:选择2022年1月—2023年1月收治的骶尾部藏毛窦患者75例,按术式不同分为A组(18例)、B组(32例)和C组(25例)。A组采用病灶切除敞开联合负压封闭引流术,B组采用保留皮桥部分缝合联合负压封闭引流术,C组采用Limberg转移皮瓣联合封闭负压引流术。比较三组围手术期指标、并发症发生率及复发率。结果:A组和B组的手术时间、拔管时间和住院时间短于C组,出血量少于C组,差异均有统计学意义(P<0.05),而A组和B组以上项目比较,差异无统计学意义(P>0.05)。伤口愈合时间比较,A组长于B组,B组长于C组,差异有统计学意义(P<0.05)。三组中A组视觉模拟评分(VAS)高于B组和C组,差异有统计学意义(P<0.05)。三组复发率比较,差异无统计学意义(P>0.05)。三组并发症发生率比较,差异无统计学意义(P>0.05)。结论:保留皮桥部分缝合联合负压封闭引流术治疗骶尾部藏毛窦具有手术时间短、出血少、术后疼痛少、住院时间短、并发症少等优点。Objective:To analyze the effect of preserved bridge suture combined with vacuum assisted closure in the treatment of sacrococcygeal pilose sinus.Methods:All 75 patients with sacrococcygeal sinus collected from Jiangmen Central Hospital from January 2022 to January 2023 were selected and divided into three groups according to different operation methods.Lesion resection and open combined with vacuum assisted closure(group A),preserved bridge partial suture combined with Vacuum assisted closure(group B),Limberg transfer flap combined with vacuum assisted closure(group C),18 cases in group A,32 cases in group B,and 25 cases in group C.The perioperative indexes,complications and recurrence rate of the three groups were compared.Results:The surgical time,extubation time,and hospitalization time of Group A and Group B were shorter than those of Group C,and the bleeding volume was less than that of Group C,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups B(P>0.05).The difference in wound healing time between group A and group B was statistically significant(P<0.05).Among the three groups,the visual analogue scale(VAS)in Group A was significantly higer than that in group B and group C,with a statistically significant difference(P<0.05),while the difference in recurrence rate was not statistically significant(P>0.05).There was no statistically significant difference in overall complications among the three groups(P>0.05).Conclusion:The treatment of sacrococcygeal pilose sinus with preserved bridge partial suture combined with vacuum assisted closure has the advantages of short operation time,less bleeding,less postoperative pain,short hospital stay and fewer complications.
