摘 要:目的:评估在重症监护室(ICU)早期标准化康复运动中加入股四头肌电刺激联合床上踏车训练的康复治疗对ICU危重患者全身肌力的改善效果。方法:选择2021年1月1日—2021年12月31日ICU收治的87例患者为对照组,2022年1月1日—2022年12月31日ICU收治的72例患者为观察组。对照组实施ICU标准化早期康复治疗,观察组在对照组基础上实施股四头肌电刺激联合床上踏车训练。对比两组患者肌肉力量、自主活动能力、自理能力、机械通气时间、ICU治疗时间以及总住院时间。结果:观察组英国医学研究委员会(MRC)评分、ICU活动量表(IMS)评分、Barthel指数高于对照组,差异有统计学意义(P<0.05);观察组机械通气时间、ICU治疗时间、总住院时间均短于对照组,差异有统计学意义(P<0.05)。结论:在ICU标准化的早期康复计划中增加早期床上踏车训练和股四头肌电刺激可以改善患者出院时的肌肉力量、活动能力和自理能力,缩短机械通气时间、ICU治疗时间和总住院时间,效果优于单纯常规康复训练。Objective:To evaluate the improvement effect of early standardized rehabilitation exercise in ICU combined with quadriceps electrical muscle stimulation and treadmill training on the whole body muscles of critically ill patients in ICU.Methods:Eighty-seven patients in ICU from January 1 to December 31 in 2021 were treated as control group,72 patients in ICU from January 1 to December 31 in 2022 were treated as control group.The control group was treated with standardized early rehabilitation therapy in ICU,and the observation group was treated with quadriceps femoris electrical stimulation combined with treadmill training on the bed.The muscle strength,self-activity,self-care ability,mechanical ventilation time,ICU treatment time and total hospitalization time were compared between the two groups.Results:The MRC muscle strength score,ICU mobility scale(IMS)score and Barthel index in the intervention group were higher than those in the control group,the difference was statistically significant(P<0.05).The mechanical ventilation time,ICU treatment time and total length of stay in the intervention group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion:Adding early bed treadmill training and quadriceps muscle electrical stimulation to standardized early rehabilitation plan in ICU can improve the muscle strength,mobility and self-care ability of patients at discharge,shorten the mechanical ventilation time,ICU treatment time and total length of stay.The effect is better than that of conventional rehabilitation training alone,and can be used in clinical promotion.
