基于综合指数法和RSR法的甘肃省急性弛缓性麻痹病例监测质量评价

摘　　要：目的评价2021年甘肃省13个市(州)急性弛缓性麻痹(acute flaccid paralysis,AFP)病例监测工作质量。方法选择AFP病例监测中报告发病率、48 h内调查率、合格便采集率、粪便标7 d内送达率、75 d随访及时率和首诊报告率等6项主要指标,运用综合指数法和秩和比法(rank-sum ratio,RSR)评价各市(州)AFP病例监测质量。结果综合指数排名前3位的为定西市(8.139)、张掖市(7.693)和兰州市(6.863);RSR法评价显示,AFP病例监测质量处于好、中、差的市(州)分别为2个、9个、2个,其中兰州市和张掖市为好档,庆阳市和陇南市为差档,其余9个市(州)为中档。结论甘肃省各市(州)AFP病例监测质量存在差异;应通过AFP专项培训、多次业务指导、与医院及病例之间加强沟通等措施提高薄弱市(州)AFP病例监测能力。Objective To evaluate the quality of acute flaccid paralysis(AFP)surveillance in 13 cities(prefectures)of Gansu province in 2021.Methods The key indicators of reported incidence rate,investigation rate with 48 hours,qualified feces collection rate,delivery rate of fecal specimens within 7 days,timely follow-up rate within 75 days and first diagnosis reporting rate were selected for evaluating the quality of AFP case surveillance in every city(prefecture)with comprehensive index method and rank-sum ratio(RSR).Results Dingxi(8.139),Zhangye(7.693)and Lanzhou(6.863)were the top 3cities as the comprehensive index results.The results of RSR showed that there were 2,9 and 2 cities(prefectures)at the level of good,medium and poor quality of AFP cases surveillance,respectively.Among them,the cities of Lanzhou and Zhangye were at the good level,Qingyang and Longnan were at the poor level,and all other 9 cities(prefectures)were at the medium level.Conclusions There were differences in the quality of AFP case surveillance among different cities(prefectures)in Gansu province.The capacity of AFP case surveillance should be improved by the special AFP training,multiple professional guidance,and enhanced by the communication among hospitals and cases in the cities(prefectures)which the capacity are weak.
