河南省濮阳市一起人感染H5N6禽流感流行病学及病原学特征分析

摘　　要：目的分析2022年3月河南省濮阳市首例人感染H5N6禽流感流行病学特征,探索感染来源,为防控人感染禽流感提供科学依据。方法对人感染H5N6禽流感病例、密切接触者和涉禽外环境开展流行病学调查,采集病例和密切接触者的呼吸道标本、环境标本、禽类生物样本和禽类产品,运用实时荧光定量PCR方法检测禽流感病毒核酸。结果该病例肺泡灌洗液、咽拭子和鼻拭子样本均检出H5N6亚型禽流感病毒核酸,病例有宰杀禽类职业暴露史。1份屠宰厂的案板涂抹拭子检出H5N6亚型禽流感病毒核酸,9份鸭头类产品的口咽拭子检出H5亚型禽流感病毒核酸,确诊为人感染H5N6禽流感,为单个散发病例。结论濮阳市首例人感染H5N6禽流感病例为单个散发病例,病原甲型H5N6亚型禽流感病毒可能来自宰杀的鸭子;应加强禽流感日常监测及健康科普。Objective To analyze the epidemiological characteristics of the first patient of human infection with H5N6 avian influenza in Puyang city of Henan province in March 2022,and explore the source of infection so as to provide scientific basis for the prevention and control of human infection with avian influenza.Methods The epidemiological investigation was carried out on the patient of human infection with H5N6 avian influenza,close contacts and external environment involving poultry.The samples from respiratory tract of patient and close contacts,external environmental,biological samples of poultry and products of poultry were collected for detecting the nucleic acid of avian influenza virus by real-time fluorescence quantitative PCR.Results The nucleic acid of avian influenza virus of subtype H5N6 was detected in alveolar lavage fluid,pharyngeal swab and nasal swab of the patient who had the occupational exposure history of poultry slaughter.The nucleic acid of avian influenza virus of subtype H5N6 was detected in 1 anvil plate smear sample that was collected from slaughterhouse,and nucleic acid of avian influenza virus of subtype H5 was detected in 9 samples of oropharyngeal swab from duck head products.This patient was a single sporadic case confirmed with human infection of subtype H5N6 avian influenza.Conclusions The first patient was a single sporadic case with human case of H5N6 avian influenza in Puyang city.The pathogen of avian influenza A subtype H5N6 may be derived from slaughtered ducks.The routine surveillance of avian influenza and health education should be strengthened.
