摘　　要：行医的本质在于回应他人由疾病造成的身心痛苦,而医生对患者的情感支持具有促成医患信任、改善医患关系、提高就医体验的积极功能。基于儿童血液病诊疗中心的参与观察研究发现,医生会根据儿童的年龄阶段和性格特征采取不同的情感支持策略,以缓解患儿由躯体症状、创伤性操作、治疗副作用引发的疼痛恐惧、形象焦虑、孤独无助等负性情绪,且儿童自身也能通过这种情感交流表达出对医生的信赖与感谢,从而促进医患信任的结成。重视医患沟通的情感维度,对反思当下心理学等学科的医患信任与社会信任研究具有理论启发意义,也可为安宁疗护和情感护理等医护模式的实践推进提供实践参照。The essence of medical practice is to respond to the physical and mental pain of others caused by diseases,in which the emotional support from doctors to patients plays a key role in promoting doctor-patient trust,improving doctor-patient relationship,and enhancing medical experience.Based on a participant observation study conducted in a children blood disease center in China,this paper finds that doctors adopt different emotional support strategies according to children's ages and personality characteristics to relieve their pain-related fear,body-image anxiety caused by somatic symptoms,traumatic operations,and treatment side effects,as well as loneliness and helplessness and other negative emotions.Simultaneously,patients can also express their trust and gratitude to the doctor through this emotional communication.Emphasizing the emotional dimension of doctor-patient communication has theoretical inspiration for reflecting on the current research on doctor-patient trust and social trust for psychology and other disciplines.It can also provide a practical reference for the practice of medical and nursing models such as palliative treatment and emotional nursing care.
