基于“伏邪”理论探讨桥本氏甲状腺炎的防治体会

摘　　要：桥本氏甲状腺炎尚未建立完整的中医诊疗体系,具有起病隐匿,发展缓慢,病程较长的特点,临床表现主要以实验室指标TPOAb、Tg Ab升高为主,严重者出现甲状腺弥漫性肿大,与中医理论“伏邪”相符。随着后世医家学术体系的不断完善,伏邪为藏匿之邪、不显于外,正气不足、邪气得伏,伏为阴邪,积聚难愈等多方面体现出其与桥本氏甲状腺炎的发病特点相关。基于伏邪理论出发,治疗本病应见微知著,先安未受邪之地,标本兼顾,衷中参西,识态定靶,深入理解伏邪学说思想,对桥本氏甲状腺炎的临床防治有所裨益。A complete TCM diagnosis and treatment system for Hashimoto's thyroiditis has not yet been established.Some patients have no obvious symptoms at first,displaying the characteristics of a hidden onset,a slow progression and a long disease course.It is clinically manifested mainly by the increase of laboratory indexes such as TPOAb and TgAb,and a diffuse goiter in severe cases,which is consistent with the theory of"latent pathogens"in Chinese medicine.With the continuous improvement of the academic system of physicians in later generations,latent pathogens have been recognized to be invisible from the exterior,due to a lack of vital qi and an excess of yin evils,related to the characteristics of Hashimoto's thyroiditis in many aspects and difficult to cure.Nowadays,traditional Chinese medicine has gained some specific advantages in the prevention and treatment of Hashimoto's thyroiditis.Based on the theory of latent pathogens,it is argued that this disease should be treated by taking into account both its manifestations and root causes,integrating Chinese medicine with western medicine,identifying its state and setting its targets,which is believed to be beneficial to the clinical prevention and treatment of Hashimoto's thyroiditis.
