滋肾育胎丸合毓麟珠“从气论治”卵巢早衰的临床疗效及对血清AMH、INHB水平的影响

摘　　要：目的探究滋肾育胎丸合毓麟珠“从气论治”卵巢早衰(POF)的临床疗效及对血清抗苗勒氏管激素(AMH)、抑制素B(INHB)水平的影响。方法选取2020年3月—2021年9月医院收治的92例POF患者作为研究对象,按照随机数字表法按1∶1比例分为联合组、常规组,各46例。常规组采用激素序贯治疗治疗,联合组采用滋肾育胎丸合毓麟珠治疗。比较两组疗效、治疗前后中医证候积分、卵巢功能[卵巢体积、双侧卵巢窦卵泡数(AFC)、子宫内膜厚度]、性激素水平[雌二醇(E_(2))、孕酮(P)、促卵泡素(FSH)、促黄体生成素(LH)]、血清AMH、INHB水平及不良反应发生率。结果联合组总有效率91.30%(42/46)较常规组73.91%(34/46)高(P<0.05);治疗后联合组中医证候积分较常规组低(P<0.05);治疗后联合组卵巢体积较常规组大,子宫内膜厚度较常规组厚,AFC较常规组多(P<0.05);治疗后联合组E2较常规组高,P、FSH、LH较常规组低(P<0.05);治疗后联合组血清AMH、INHB水平较常规组高(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论滋肾育胎丸合毓麟珠“从气论治”POF效果显著,可有效减轻临床症状,提高卵巢储备功能,调节性激素及血清AMH、INHB水平,安全性高。Objective To explore the clinical efficacy of Zishen Yutai Pill(滋肾育胎丸)and Yulin Zhu(毓麟珠)in treatment of premature ovarian failure(POF)from the perspective of Qi and its effect on serum levels of anti-Müllerian hormone(AMH)and inhibin B(INHB).Methods A total of 92 patients with POF who were admitted to the hospital from March 2020 to September 2021 were selected as the research subjects,and were divided into the combined group and the conventional group according to the random number table method according to the ratio of 1:1,with 46 cases in each group.The conventional group was treated with sequential hormone therapy,and the combined group was treated with Zishen Yutai Pill and Yulin Zhu.The curative effect,TCM syndrome scores before and after treatment,ovarian function[ovarian volume,number of antral follicle(AFC),endometrial thickness],sex hormone levels[estradiol(E_(2)),progesterone(P),Follicle-stimulating hormone(FSH),luteinizing hormone(LH)],serum levels of AMH and INHB and incidence of adverse reactions of the two groups were compared.Results The total effective rate of the combined group was 91.30%(42/46),higher than that of the conventional group(73.91%,34/46)(P<0.05).After treatment,the TCM syndrome score of the combined group was lower than that of the conventional group(P<0.05).The intimal thickness was thicker than that of the conventional group,and the AFC was higher than that of the conventional group(P<0.05).INHB levels were higher than those in the conventional group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Zishen Yutai Pill combined with Yulin Zhu has a significant effect on POF and can effectively relieve the clinical symptoms,improve the ovarian reserve,regulate sex hormones and serum levels of AMH and INHB,with high safety.
