经阴道免气腹腔镜下子宫全切术的临床应用效果

摘　　要：目的分析经阴道免气腹腔镜下子宫全切术的应用效果。方法选择156例需行子宫全切术的患者为研究对象,根据随机数字表法分为对照组与研究组,每组78例。对照组给予传统气腹腹腔镜手术,研究组经阴道行免气腹腹腔镜手术治疗,比较两组手术指标、血压、血气指标及并发症情况。结果研究组手术时间长于对照组,术中出血量多于对照组,术后排气时间及住院时间均短于对照组(P<0.05);两组术中血压与动脉血氧饱和度(SaO_(2))比较,差异均无统计学意义(P>0.05);研究组动脉血氧分压(PaO_(2))、二氧化碳分压(PCO_(2))均优于对照组(P<0.05);研究组术后并发症发生率低于对照组(P<0.05)。结论经阴道免气腹腔镜下子宫全切术对患者影响小,住院时间短,并发症发生率低,值得临床推广与应用。Objective To analyze the effect of total hysterectomy by transvaginal airless laparoscopy.Methods A total of 156 patients needing total hysterectomy were selected as the study objects and divided into control group and study group according to random number table method,78 cases in each group.The control group was given traditional pneumoperitoneoscopy,and the study group was given transvaginal pneumoperitoneotomy without pneumoperitoneoscopy.The surgical indexes,blood pressure,blood gas indexes and complications of the two groups were compared.Results The operation time of the study group was longer than that of the control group,the amount of blood loss during operation was more than that of the control group,and the postoperative exhaust time and hospital stay were shorter than that of the control group(P<0.05).There was no significant difference in intraoperative blood pressure and arterial oxygen saturation(SaO_(2))between the two groups(P>0.05).Partial pressure of arterial oxygen(PaO_(2)),partial pressure of carbon dioxide(PCO_(2)),in the study group were better than those in the control group(P<o.05).The incidence of postoperative complications in the study group was lower than that in the control group(P<o.05).Conclusion Total hysterectomy by transvaginal air-free laparoscopy has little effect on patients,short hospital stay and low complication rate,which is worthy of clinical promotion and application.
