第一产程自由体位护理联合无痛分娩对分娩结局及产程时间的影响

摘　　要：目的探讨产妇应用第一产程自由体位护理联合无痛分娩对分娩结局和产程时间的影响。方法选取66例分娩产妇为研究对象,根据随机数字表法分为对照组和观察组,每组33例。对照组采用常规体位护理+无痛分娩,观察组采用第一产程自由体位护理+无痛分娩,比较两组产程时间、疼痛情况及分娩结局。结果观察组第一产程、第二产程、第三产程及总产程时间均短于对照组(P<0.05);观察组产妇自然分娩率高于对照组(P<0.05);观察组中转剖宫产率及新生儿窒息率低于对照组(P<0.05);两组产钳助产率比较,差异无统计学意义(P>0.05);观察组疼痛评定指数(PRI)、现实疼痛的强度(PPI)、视觉模拟量表(VAS)评分均低于对照组(P<0.05)。结论应用第一产程自由体位护理+无痛分娩,能够加快产妇分娩进程,提高自然分娩率,减少分娩疼痛感,对保障母婴健康有积极意义,值得临床推广与应用。Objective To investigate the effects of free postural nursing in the first stage of labor and painless labor on labor outcome and labor time.Methods A total of 66 cases of parturient women were selected as research objects and divided into control group and observation group according to random number table method,33 cases in each group.The control group received conventional postural nursing+painless delivery,and the observation group received free postural nursing+painless delivery in the first stage of labor.The labor time,pain and delivery outcome of the two groups were compared.Results The first stage of labor,the second stage of labor,the third stage of labor and the total stage of labor in the observation group were shorter than those in the control group(P<o.05).The natural delivery rate in the observation group was higher than that in the control group(P<o.05).The rates of cesarean section and neonatal asphyxia in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the rate of forceps delivery between the two groups(P>0.05).The scores of Pain Rating Index(PRI)、Psychological PainIntensitly(PPI)and Visual AnalogScale(VAS)inobservationgroup were lower than those in control group(P<0.05).Conclusion The application of free position nursing+painless delivery in the first stage of labor can speed up the process of childbirth,improve the rate of natural delivery,reduce the pain of childbirth,and has positive significance for the protection of maternal and infant health,which is worthy of clinical promotion and application.
