根本原因分析法在骨科植入器械清洗消毒过程中的应用

摘　　要：目的:探讨根本原因分析法在骨科植入器械清洗消毒过程中的临床应用价值。方法:选择2020年1月~2021年5月本院消毒供应室接受的400件骨科植入器械为研究对象。随机分为观察组与对照组,各200件。对照组行供应室常规管理,观察组在常规清洗消毒基础上,使用根本原因分析法进行管理,比较两组清洗质量、消毒合格率、供应室满意度,统计两组供应室相关不良事件发生情况,比较两组单个器械包运行成本比较。结果:观察组清洗质量合格率、消毒合格率、供应室满意度均显著高于对照组(P<0.05),观察组发生器械不符、无菌包标签错误、消毒不合格的总比例显著低于对照组(P<0.05)。观察组单个器械包运行成本中,消毒灭菌成本、运输成本和管理成本均显著低于对照组(P<0.05)。结论:针对骨科植入手术的清洗消毒过程,使用根本原因分析法,能有效地提高清洗质量合格率、消毒合格率、供应室满意度,降低不良事件发生率和医疗成本。Objective:To explore the clinical application value of root cause analysis in the cleaning and disinfection process of orthopedic implant devices.Methods:400 orthopedic implant devices were selected as research objects from January 2020 to May 2021 were randomly divided into two groups,each with 200 pieces.The control group was managed by routine cleaning and disinfection,while the observation group was managed by root cause analysis based on routine cleaning and disinfection.Then the cleaning quality,disinfection qualification rate and satisfaction degree of the supply room were compared between the two groups.The incidence of adverse events related to the supply room of the two groups was counted,and the operating cost of a single set was compared between the two groups.Results:The qualified rate of cleaning quality,disinfection and satisfaction of supply room in the observation group were significantly higher than those in the control group(P<0.05),while the total proportion of equipment discrepancy,aseptic package label error and unqualified disinfection in the observation group was significantly lower than that in the control group(P<0.05).Among the operating costs of a single set in the observation group,the disinfection and sterilization costs,transportation costs and management costs were significantly lower than those in the control group(P<0.05).Conclusion:According to the cleaning and disinfection process of orthopedic implant surgery,the intervention of root cause analysis can effectively improve the qualified rate of cleaning quality,disinfection and satisfaction of supply room,and reduce the incidence of adverse events and medical costs.
