摘　　要：目的对四川某三甲医院康复科患者住院费用的变化和结构进行分析,为有效控制康复科患者住院费用及减轻康复患者经济负担提供参考和数据支持。方法通过医院信息系统收集该医院康复科2013—2021年所有患者的住院费用,利用灰色关联模型和结构变动度分析该院康复科费用的影响因素和费用结构。结果从康复患者住院费用看,2016年前较平稳,2017年后逐年增高,可能由于新冠肺炎的影响,2020及2021年次均费用有所下降。从关联度看,治疗费(0.887)、材料费(0.695)、西药费(0.688)和检查费(0.686)4项指标是该院康复科次均住院费用的主要影响因素;而输氧费(0.653)、麻醉费(0.651)、其他(0.649)等其他相关因素关联度则相对最低。从结构变动值和变动度看正向变动的前三位依次为检查费(4.44)、材料费(3.74)和检验费(2.59),负向变动绝对值前三位依次是治疗费(-6.42)、西药费(-4.06)和床位费(-1.41)。结论在老龄化程度持续加深的背景下,康复治疗的住院费用逐年增大,治疗费、材料费、西药费等是影响次均住院费用主要因素,此外检查和检验费用的影响不容忽视。应该优化康复医疗费用的构成、规范严格的康复临床路径、合理体现劳动力价值,从而切实减轻康复患者经济负担。Objective To analyze the hospitalization cost structure of patients in the rehabilitation department of a third-class A hospital in Sichuan province,and to provide reference and data support for effectively controlling the hospitalization cost and reducing the economic burden of rehabilitation patients.Methods The hospital information system was used to collect the hospitalization expenses of all patients in the rehabilitation department of the hospital from 2013 to 2021,and the influencing factors and cost structure of the rehabilitation department of the hospital were analyzed through the grey correlation model and structural change analysis.Results From the perspective of inpatient costs of rehabilitation patients,they were relatively stable before 2017,and increased year by year after 2017.The average cost per time in 2020 and 2021 may be reduced due to the impact of COVID-19.From the perspective of correlation,the four indicators of treatment cost(0.887),material cost(0.695),western medicine cost(0.688)and examination cost(0.686)were the main influencing factors of the average hospitalization cost of the rehabilitation department of the hospital.The correlation degree of other related factors,such as oxygen transfusion cost(0.653),anesthesia cost(0.651)and others(0.649),was relatively low.From the perspective of structural change value and degree of change,the first three places of positive change were inspection cost(4.44),material cost(3.74)and test cost(2.59),and the first three places of negative absolute change were treatment cost(-6.42),western medicine cost(-4.06)and bed cost(-1.41).Conclusion In the context of the deepening of aging,the cost of rehabilitation treatment is increasing year by year.The cost of treatment,materials and western medicine are still the main factors that affect the average hospitalization cost.In addition,the impact of examination and laboratory costs cannot be ignored.The composition of rehabilitation medical expenses should be optimized,the standardized and strict rehabilit
