摘　　要：目的基于结构变动度对长治市高发的五种恶性肿瘤患者住院费用进行变化趋势分析,从而为深化药品集中带量采购制度改革政策落实情况提供依据。方法收集长治市所有医保定点医院2018—2020年的五种恶性肿瘤患者住院费用,利用结构变动度的方法,通过计算相应次均医药费结构变动相关数值,来考察相关的构成变化趋势。结果2018—2020年间,住院费用总的结构变动度为25.35%,药费、诊断费、治疗费和材料费呈下降趋势,综合医疗服务费、血液和血制品费用呈上升趋势,在所有恶性肿瘤住院费用中,治疗费的结构贡献率最大,其次为材料费。医药费关联度排名前三项分别是药费(1.0000)、材料费(0.9684)、治疗费(0.9483)。结论恶性肿瘤住院费用的相关政策落实情况初有成效,政府应进一步加大政策落实力度,进一步降低恶性肿瘤患者药费负担。Objective To analyze the trend of changes in hospitalization costs of patients with five types of malignant tumors with high prevalence in Changzhi city based on the degree of structural change,so as to provide a basis for deepening the implementation of the reform policy of centralized quantity purchasing system for drugs.Methods The hospitalization costs of patients with five types of malignant tumors in all medical insurance designated hospitals in Changzhi City from 2018 to 2020 were collected,and the trend of related composition changes was examined by calculating the values related to the structural changes of the corresponding sub-average medical costs using the method of structural change degree.Results The total structural change degree of hospitalization costs during 2018—2020 was 25.35%,with a decreasing trend in drug,diagnostic,treatment and material costs,and an increasing trend in comprehensive medical services,blood and blood products costs,and the largest structural contribution of treatment costs among all malignancy hospitalization costs,followed by material costs.The top three items of medical cost correlation were drug cost(1.0000),material cost(0.9684),and treatment cost(0.9483).Conclusion The implementation of policies related to malignant tumor hospitalization costs has been effective at first,and the government should further increase the implementation of policies to further reduce the burden of drug costs for malignant tumor patients.
