Nice结在骨筋膜室综合征切开减压伤口中的临床应用研究

摘　　要：目的通过Nice结与传统缝合方法在骨筋膜室综合征(筋膜间隔综合征)切开减压外科伤口的治疗比较,探讨Nice结在骨筋膜室综合征切开减压伤口中的应用效果。方法选取2012年3月至2022年4月收治的骨筋膜室综合征切开减压患者27例,根据手术方式将患者分为Nice结组14例,传统缝合组13例。比较两组患者的缝合次数(切开减压术后至创面闭合)、切开减压术至创面闭合所需天数、温哥华瘢痕量表(Vancouver Scar Scale,VSS)评分和创面瘢痕优良率。结果两组均创面愈合,其中Nice结组患者未植皮,传统缝合组患者3例植皮。两组患者VSS评分、创面瘢痕优良率、切开减压术至创面闭合所需天数差异无统计学意义。Nice结组缝合次数显著低于传统缝合组(P<0.05),差异有统计学意义。结论Nice结治疗骨筋膜室综合征切开减压伤口可以减少缝合次数;且可以避免植皮。因此可以替代传统缝合方法治疗骨筋膜室综合征切开减压伤口。Objective To explore the application value of Nice knot by comparing it with traditional suture method in the treatment of osteofascial compartment syndrome.Methods A total of 27 patients with osteofascial compartment syndrome and incision compression were selected from March 2012 to April 2022.According to the wound closure technique,the patients were divided into 2 groups.Nice knot was applied in patients in group A(14 cases),while traditional suture method in group B(13 cases).Vancouver Scar Scale(VSS)score,excellent and good rate of wound scar,suture frequency and the time from incision decompression to wound closure were compared between the two groups.Results Wound healing was achieved in both groups.None of the patients in group A had skin grafting,and 3 patients in group B needed skin grafting.There were no signiflcant differences in VSS score,the excellent and good rate of wound scar and the time from incision decompression to wound closure.Suture frequency in group A was signiflcantly lower than those in group B(P< 0.05).Conclusions The application of Nice knot could reduce suture frequency and avoid skin grafting,which is recommended in the treatment of osteofascial compartment syndrome.
