摘 要:目的:调查分析能量型器械碳化后的焦痂清除现状,为规范能量型器械碳化后焦痂清除管理提供参考。方法:采用问卷星调研方式针对能量型器械碳化后焦痂处理状况的一般信息部分、能量型器械术中使用焦痂处理情况和医院消毒供应中心(CSSD)集中管理能量型器械情况,并对江苏省内的180家医院(综合型医院153家,专科医院27家)能量型器械碳化后焦痂清除现状进行调查。结果:共收回有效问卷180份,涉及180家医院的180名调查对象。180家医院中设置能量型器械处理岗位专岗制度的132家(占73.33%),术中有降温措施的137家(占76.11%),术中清除焦痂的151家(占83.89%),使用后立即保湿的95家(占52.78%)。180家医院焦痂清除方法多选项中用克氏针剔除的165家(占91.67%),超声清洗使用高频超声的87家(占48.33%),机械清洗时选择减压沸腾清洗方式的45家(占25%),焦痂清除效果评价达标仅有51家(占28.33%)。能量型器械处理岗位专岗制度、术后是否及时处理和建立标准作业流程是能量型器械处理质量的重要影响因素(OR=1.675,OR=1.900,OR=0.487;P<0.05)。结论:CSSD应设置能量型器械处理专职岗位,提高能量器械术中使用规范,加强术后有效及时处理,合理使用清洗及清洗质量监测方法,降低医院感染风险,提高医疗护理质量。Objective:To investigate the current status of carbonized eschar removal on energy-based instruments in Central Sterile Supply Department(CSSD)of hospitals in Jiangsu Province,and to provide reference for standardizing the management of carbonized eschar removal of energy-based instruments.Methods:The investigation questionnaire on the eschar treatment after carbonization of energy-based devices includes three parts:general information,eschar treatment status during the use of energy-based instruments during surgery and the centralized management of energy instruments by CSSD.A questionnaire survey was conducted to investigate the current status of carbonized eschar removal from energy-based instruments in hospitals in Jiangsu Province by Wenjuanxing.Results:A total of 180 valid questionnaires were collected,involving 180 respondents from 180 hospitals in Jiangsu Province.Among the 180 surveyed hospitals,132(accounting for 73.33%)had a dedicated post system for energy-based device processing,137(accounting for 76.11%)with cooling measures during operation,151(accounting for 83.89%)with eschar removal during operation,and 95(accounting for 52.78%)with moisturizing immediately after use.Among the multiple options for eschar removal in 180 hospitals surveyed,165(accounting for 91.67%)used Kirschner wire to remove eschar,87(accounting for 48.33%)used high-frequency ultrasound for ultrasonic cleaning,45(accounting for 25%)used reduced pressure boiling cleaning for mechanical cleaning,and only 51(accounting for 28.33%)met the standard for eschar removal effect evaluation.The position-specific system for energy-based instrument processing,timely treatment after surgery and the establishment of standard operating procedures were important factors affecting the quality of energy-based instrument processing(OR=1.675,OR=1.900,OR=0.487;P<0.05).Conclusion:CSSD should set up full-time positions for the treatment of energy instruments,improve the specifications for the intraoperative use of energy instruments during operation,s
