预防性应用鼻腔冲洗及糠酸莫米松鼻喷剂对变应性鼻炎疗效的影响

摘　　要：目的探究在变应性鼻炎症状发作前预防应用糠酸莫米松鼻喷剂与鼻腔冲洗能否提高整体疗效。方法选取该院2021年8-9月经门诊确诊的146例中重度AR患者为研究对象。以是否在花粉播散高峰前2周进行预防分为预防组和对照组,每组73例。两组受试者均于AR症状发作期接受糠酸莫米松鼻喷剂联合鼻腔冲洗治疗。在治疗前、治疗2周及4周应用鼻结膜炎相关生活质量问卷(RQLQ)、鼻部症状视觉模拟量表(VAS)及临床体征评分进行评估。结果RQLQ总分、鼻部总症状视觉模拟量表(Uni-VAS)评分及鼻内镜下体征评分在组间(F=7.955,P=0.005;F=4.615,P=0.033;F=10.719,P=0.001)及不同时间节点间(F=22.347,P<0.001;F=8.154,P<0.001;Greenhouse-Geisser校正系数为0.903,P<0.001),差异均有统计学意义。分组与时间节点间均无交互作用(F=1.666,P=0.191;F=0.079,P=0.924;F=0.389,P=0.657)。结论联合应用糠酸莫米松鼻喷剂与鼻腔冲洗于AR症状发作前进行预防,有利于提高AR的整体疗效。Objective To investigate whether the preventive application of mometasone furoate nasal spray and nasal irrigation can improve the overall therapeutic effect of allergic rhinitis(AR).Methods A total of 146 patients with moderate to severe AR diagnosed in the outpatient clinic from August to September 2021 were selected as the study subjects and divided into the prevention group and control group according to whether conducting the prevention in 2 weeks before the peak of pollen dispersal,73 cases in each group.All subjects in the two groups were treated with mometasone furoate nasal spray and nasal irrigation in the onset of AR symptoms.The RQLQ,nasal symptom VAS and clinical signs scores were used to conduct the evaluation before treatment,as well as in 2,4 weeks after treatment.Results The RQLQ total score,Uni-VAS score and signs score under endoscope had the statistical differences between the groups(F=7.955,P=0.005;F=4.615,P=0.033;F=10.719,P=0.001)and between the different time nodes(F=22.347,P<0.001;F=8.154,P<0.001;the Greenhouse Geisser correction coefficient=0.903,P<0.001).There was no the interaction in the groups and time nodes(F=1.666,P=0.191;F=0.079,P=0.924;F=0.389,P=0.657).Conclusion The combined application of mometasone furoate nasal spray and nasal irrigation for conducting the prevention before AR symptoms onset is conducive to increase the AR overall treatment effect.
