摘　　要：自身免疫性疾病(AIDs)与恶性肿瘤均属于临床难治疾病,二者之间存在交织关系,两种疾病共病促使患者死亡率大大增加,当务之急是了解两者疾病发展的根本原因和具体的分子机制。一方面,AIDs中自身特异性抗体、炎性因子释放、免疫失调及治疗AIDs应用的药物均可能导致恶性肿瘤的发生。另一方面,恶性肿瘤的免疫检查点抑制剂也是导致两种疾病共病的机制之一。因此,对于两种疾病共病的情况,需要风湿科和肿瘤科医师在诊断时保持谨慎,并保持密切的跨学科合作,剖析共病患者的主要病机,权衡两种疾病共病轻重,择取最佳治疗方式尤为关键。该文就自身免疫性疾病与恶性肿瘤共病机制的研究进展做一综述。Autoimmune diseases(AIDs)and malignant tumors are both clinically refractory diseases,and there is an interwoven relationship between the two.The comorbidity of the two diseases greatly increases the mortality rate of patients.It is urgent to understand the root causes and specific molecular mechanisms of the development of the two diseases.On the one hand,in AIDs,the release of autospecific antibodies,inflammatory factors,immune imbalance and drugs in treating AIDs may lead to the occurrence of malignant tumors.On the other hand,the immune checkpoint inhibitors of malignant tumors are also one of the mechanisms leading to the comorbidity of the two diseases.Therefore,for the comorbidity of two diseases,it is necessary for rheumatologists and oncologists to be cautious in diagnosis and maintain close interdisciplinary cooperation,analyze the main pathogenesis of the patients with comorbidity,weigh the severity of comorbidity of the two diseases,and choose the best treatment mode.This article reviews the research progress on the comorbidity mechanism of autoimmune diseases and malignant tumors.
