高频超声结合弹性成像在强直性脊柱炎治疗前后跟腱的诊断评估

摘　　要：目的采用高频超声结合弹性成像技术观察强直性脊柱炎(AS)患者治疗前后的跟腱图像特点,并探讨其诊断价值。方法对30例AS患者进行综合药物及康复治疗2年,治疗前后均进行跟腱高频超声及弹性成像检查,高频超声测量跟腱的近、中、远段的直径,评估肌腱的回声结构,并检查附着点处有无其他病变。弹性成像均测量近、中、远段的剪切波速度(SWV)。结果高频超声测量中段的跟腱直径和宽厚比具有统计学意义(P<0.05),近、远段的跟腱直径和宽厚比则没有统计学意义(P>0.05)。此外,弹性成像结果显示,治疗前后SWV在近、中、远三段均具有统计学意义(P<0.05)。结论高频超声结合弹性成像在AS患者跟腱治疗前后的评估中具有较高的诊断价值。Objective To observe the imaging features of the Achilles tendon in patients with ankylosing spondylitis(AS)before and after treatment using high-frequency ultrasound combined with elastography technology,and to explore its diagnostic value.Methods Thirty patients with AS were treated with comprehensive medication and rehabilitation for 2 years.Both before and after treatment,high-frequency ultrasound and elastography of the Achilles tendon were performed.The diameter of the proximal,middle,and distal segments of the Achilles tendon was measured using high-frequency ultrasound,and the echo structure of the tendon was evaluated.Other lesions were also examined at the attachment point.Elastic imaging measures shear wave velocity(SWV)in the near,medium,and far segments.Results The differences in the midsection diameter and width-to-thickness ratio of the Achilles tendon measured by high-frequency ultrasound were statistically significant(P<0.05),while the diameter and width-to-thickness ratio of the Achilles tendon in the near and far segments showed no statistical significance(P>0.05).In addition,elastic imaging results showed that SWV had statistical significance in the near,middle,and far segments before and after treatment(P<0.05).Conclusion The combination of high-frequency ultrasound and elastography has a high diagnostic value in the evaluation of Achilles tendon in AS patients before and after treatment.
