血必净对脓毒血症患者淋巴细胞线粒体功能的影响

摘　　要：目的观察脓毒血症患者淋巴细胞线粒体功能的变化,探讨血必净对患者淋巴细胞线粒体功能的影响。方法收集2019年12月~2022年1月于潍坊医学院附属医院住院的脓毒症患者60例,随机分为治疗组和对照组,各30例;对照组进行常规治疗,治疗组在常规治疗的基础上加用血必净注射液,对两组患者入院时、入院后1d、3d、7d的淋巴细胞线粒体膜电位、ATP酶活性、淋巴细胞凋亡率进行检测,统计两组患者28d病死率,并采用SPSS26.0软件对数据进行统计分析。结果两组患者住院天数有明显差异(P<0.05);治疗组入院后1d、3d和7d的线粒体电位、Na^(+)K^(+)ATP酶活性、Ca^(2+)Mg^(2+)ATP酶活性均显著高于对照组(P<0.05),淋巴细胞凋亡率低于对照组(P<0.05);两组患者28d病死率无明显差异(P>0.05)。结论脓毒血症导致患者淋巴细胞异常凋亡及线粒体功能异常,血必净可以减少淋巴细胞的异常凋亡,改善线粒体功能,对脓毒血症有一定的治疗作用。Objective To investigate the influence of mitochondrial function with septicopyemia and to discuss the effect of Xuebijing injection on mitochondrial protection level in sepsis patients.Methods From December 2019 to January 2022,60 patients with sepsis admitted to the ICU of the Affiliated Hospital of Weifang Medical University were randomly divided into routine treatment group and Xuebijing group,with 30 cases in each group.The mitochondrial membrane potential(MMP)and Na^(+)K^(+)ATPase,Ca^(2+)Mg^(2+)ATPase were determined at the admission,or 1 day,3 days,7 days after the admission.The rate of allcause death in 28 days were analyzed at the end.The statistical analysis were performed by using SPSS 26.0 software.Results The mitochondrial MMP of the treatment group increased,and the activity of Na^(+)K^(+)ATPase and Ca^(2+)Mg^(2+)ATPase at 1 day,3 days,7 days after adimission in the treatment group were significantly higher than those in the control group(P<0.05).The rate of lymphocyte apoptosis of the treatment group was significantly lower than that of control group at 1 day,3 days,7 days(P<0.05).Compared with the control group,days spent in hospital were shortened(P<0.05).The rates of allcause death in 28 days between two groups have no significantly difference(P>0.05).Conclusion The lymphocyte mitochondrial function was influenced in patients with septicopyemia.There are abnormal lymphocyte apoptosis in the septicopyemia patient.Xuebijing can improve the mitochondrial function and achieve a potential therapeutic effect on septicopyemia.
