摘 要:目的:探讨碳酸氢钠口腔护理对呼吸机相关性肺炎发生率的影响。方法:计算机检索中国知网(CNKI)、万方数据库、中国生物医学文献数据库(CBM)、维普数据库、PubMed、EMbase、the Cochrane Library、Web of Science等数据库公开发表的关于碳酸氢钠口腔护理对新生儿呼吸机相关性肺炎发生率影响的随机对照试验。根据Cochrane系统评价手册5.1.0版推荐的评价标准进行文献质量评价,采用RevMan 5.3软件对结局指标进行Meta分析。结果:共纳入10篇文献。Meta分析结果显示,与常规护理相比,碳酸氢钠口腔护理能降低机械通气新生儿呼吸机相关性肺炎发生率[OR=0.22,95%CI(0.13,0.36),P<0.00001]及口腔感染发生率[OR=0.15,95%CI(0.07,0.31),P<0.00001]。结论:现有证据表明,碳酸氢钠口腔护理可有效预防新生儿呼吸机相关性肺炎的发生。Objective:To investigate the effect of sodium bicarbonate oral care on the incidence of ventilator-associated pneumonia.Method:Published randomized controlled trials on the effect of sodium bicarbonate oral care on the incidence of ventilators associated pneumonia in newborns were retrieved from CNKI,WanFang Database,CBM,VIP,PubMed,EMbase,the Cochrane Library,Web of Science and other databases.Literature quality was evaluated according to the evaluation criteria recommended by the Cochrane Manual of Systematic Review 5.1.0,and Meta-analysis of outcome indicators was performed using RevMan 5.3 software.Results:A total of 10 articles were included.The results of meta-analysis showed that compared with conventional care,sodium bicarbonate oral care can reduce the incidence of ventilators associated pneumonia in mechanically ventilated newborns(OR=0.22,95%CI 0.13-0.36,P<0.00001)and reduce the incidence of oral infection(OR=0.15,95%CI 0.07-0.31,P<0.00001).Conclusion:The existing evidence shows that sodium bicarbonate oral care can effectively prevent the occurrence of ventilator-associated pneumonia in neonates.
