摘 要:目的探讨脑利尿钠肽(BNP)、超敏心肌肌钙蛋白I(hscTnI)联合中性粒细胞/淋巴细胞比值(NLR)和单核细胞/淋巴细胞比值(MLR)对慢性肾脏病(CKD)合并冠心病的诊断价值。方法比较CKD合并冠心病患者(A组,165例)和单纯CKD患者(B组,98例)的BNP、hscTnI、NLR和MLR,比较两组CKD不同分期患者的BNP、hscTnI、NLR和MLR,分析CKD合并冠心病的独立影响因素,绘制ROC曲线分析BNP、hscTnI、NLR和MLR单独和联合检测对CKD合并冠心病的诊断效能。结果与B组比较,A组BNP、hscTnI、NLR和MLR均增高(P<0.05)。两组BNP、hscTnI、NLR和MLR随CKD严重程度的增加呈升高趋势(P<0.05),A组升高趋势更明显(P<0.05)。BNP、hscTnI、NLR和MLR是CKD合并冠心病的独立影响因素(P<0.01)。BNP、hscTnI、NLR、MLR单独检测和联合检测对CKD合并冠心病具有一定的诊断效能(P<0.01)。结论BNP、hscTnI联合NLR和MLR可用于CKD合并冠心病的诊断。Objective To investigate the diagnostic value of brain natriuretic peptide(BNP),hypersensitive cardiac troponin I(hscTnI)complicated with neutrophil-to-lymphocyte ratio(NLR)and monocyte-to-lymphocyte ratio(MLR)in chronic kidney disease(CKD)complicated with coronary heart disease(CHD).Methods The BNP,hscTnI,NLR and MLR were compared between patients with CKD complicated with CHD(group A,165cases)and the patients with CKD(group B,98 cases),which of the patients in different stages of CKD were compared between the two groups.The independent influencing factors of CKD complicated with CHD were analyzed,and ROC curve was drawn to analyze the diagnostic efficacy of BNP,hscTnI,NLR and MLR alone and in combination in detecting CKD combined with CHD.Results Compared with group B,BNP,hscTnI,NLR and MLR were increased in group A(P<0.05).BNP,hscTnI,NLR and MLR were increased as the increased severity of CKD in both groups(P<0.05),which were more obvious in group A(P<0.05).BNP,hscTnI,NLR and MLR were the independent influencing factors of CKD complicated with CHD(P<0.01).BNP,hscTnI,NLR,MLR alone and combined detection had a certain diagnostic efficacy for CKD complicated with CHD(P<0.01).Conclusion BNP,hscTnI combined with NLR and MLR can be used for the diagnosis of CKD complicated with CHD.
