摘　　要：目的研究Q开关1064 nm Nd:YAG激光联合水光注射疗法及540nm强脉冲光治疗黄褐斑疗效。方法选选取中山市博爱医院从2021年3月至2023年3月收治的60例黄褐斑患者进行研究。将其以电脑编号随机法分作治疗组(n=30)及对照组(n=30)。对照组开展Q开关1064 nmNd:YAG激光治疗,治疗组则于对照组的基础上,增用水光注射疗法及540 nm强脉冲光治疗。对比两组疗效,皮肤屏障功能,皮肤肤质,皮损及皮周部位血管扩张程度。结果治疗组治疗总有效率高于对照组(96.67%vs 76.67%)(P<0.05)。治疗后两组各项皮肤屏障功能指标水平均高于治疗前,且治疗组高于对照组(P<0.05)。治疗后两组各项VISIA皮肤图像分析指标均低于治疗前,且治疗组低于对照组(P<0.05)。治疗后两组皮损部位a^(*)值均低于治疗前,且治疗组低于对照组(P<0.05)。结论1064nmNd:YAG激光联合水光注射疗法及540nm强脉冲光治疗黄褐斑的疗效较佳,可改善皮肤屏障功能及皮肤肤质,改善皮损血管扩张程度。Objective To study the efcacy of Q-switch 1064 nm Nd:YAG laser combined with water light injection therapy and 540 nm intense pulsed light in the treatment of melasma.Methods 60 patients with melasma who were admitted to Zhongshan Bo'ai Hospital from March 2021 to March 2023 were studied.They were randomly divided into treatment group(n=30)and control group(n=30)by computer numbering method.The control group received Q-switched 1064 nm Nd:YAG laser treatment,and the treatment group received water injection and 540 nm intense pulsed light treatment on the basis of the control group.The curative effect,skin barrier function,skin texture,skin lesion and blood vessel dilatation degree in pericutaneous area were compared between the two groups.Results The total effective rate of treatment group was higher than that of control group(96.67%vs 76.67%)(P<0.05).After treatment,the skin barrier function indexes in both groups were higher than before treatment,and the treatment group was higher than the control group(P<0.05).After treatment,the VISIA skin image analysis indexes in both groups were lower than before treatment,and the treatment group was lower than the control group(P<0.05).After treatment,the a^(*)value of the lesion site in both groups was lower than before treatment,and the treatment group was lower than the control group(P<0.05).Conclusion 1064 nm Nd:YAG laser combined with water light injection therapy and 540 nm intense pulse light have better curative effect in the treatment of melasma,which can improve the skin barrier function and skin texture,and improve the dilatation degree of skin vessels.
