摘 要:目的 探讨初次关节置换患者粒细胞集落刺激因子(GCSF)基因多态性与术后关节假体周围感染(PJI)易感性的关联。方法 选择四川省岳池人民医院骨科2018年12月-2021年12月初次关节置换患者为研究对象,根据术后关节假体周围感染情况分为PJI组和非PJI组,各40例。采用聚合酶链式扩增反应(PCR)法分析不同患者GCSF基因rs3917924位点携带基因型情况,调查并收集所有患者性别、年龄、体质量指数、手术部位和术前窦道情况;统计所有患者微生物培养结果、外周血血清中C-反应蛋白(CRP)、红细胞沉降率(ESR)、视觉模拟评分(VAS)。结果 哈迪-温伯格平衡(HWE)检验结果显示PJI组和非PJI组GCSF基因rs3917924位点实际频数和理论频数比较差异无统计学意义;PJI组GCSF基因rs3917924位点携带TT基因型、T等位基因频率均高于非PJI组(P<0.05);PJI组携带TT基因型患者和携带CC/CT型患者微生物培养结果、VAS评分比较差异无统计学意义,TT组外周血CRP、ESR水平高于CC/CT组(P<0.05)。结论 GCSF基因rs3917924位点携带TT基因型增加初次关节置换术后PJI风险,但与疼痛程度无关。OBJECTIVE To investigate the association of gene polymorphisms of granulocyte colony stimulating factor(GCSF) with the susceptibility to periprosthetic joint infection(PJI) in patients undergoing primary joint replacement.METHODS Patients who underwent primary joint replacement in the orthopedics department of Yuechi People′s hospital between Dec.2018 and Dec.2021 were selected as the study subjects,and were divided into PJI group and non-PJI group according to presence or absence of postoperative PJI,with 40 cases in each group.The genotypes of GCSF gene at rs3917924 locus were analyzed by polymerase chain amplification reaction(PCR).The gender,age,body mass index,surgical sites and preoperative sinus tract of all patients were investigated and collected.The results of microbial culture,levels of peripheral blood C-reactive protein(CRP) and erythrocyte sedimentation rate(ESR),and score of visual analogue scale(VAS) were statistically analyzed in all patients.RESULTS The results of Hardy-Weinberg equilibrium test showed that there was no significant difference in actual frequency and theoretical frequency of GCSF gene at rs3917924 locus between PJI group and non-PJI group.The frequencies of TT genotype and T allele in PJI group were higher than those in non-PJI group(P<0.05).There was no significant difference in results of microbial culture and VAS score between patients with TT genotype and those with CC/CT genotype.The levels of peripheral blood CRP and ESR in TT group were higher than those in CC/CT group(P<0.05).CONCLUSION TT genotype of GCSF gene at rs3917924 locus increased the risk of PJI after primary joint replacement,but was not associated with pain levels.
