摘　　要：目的 分析感染性肺炎新生儿血清淀粉样蛋白A(SAA)、血清肌钙蛋白T(cTnT)、可溶性髓样细胞触发型受体-1(sTREM-1)及其诊断价值。方法 选取2020年1月-2022年1月济南市第二妇幼保健院新生儿科住院的200例感染性肺炎新生儿作为研究对象,将其作为研究组,另外选取同期150名健康新生儿作为对照组,采用酶联免疫法检测SAA、cTnT、sTREM-1表达量,受试者工作特征(ROC)曲线分析SAA、cTnT、sTREM-1对新生儿感染性肺炎的诊断价值。结果 研究组SAA、cTnT、sTREM-1高于对照组(P<0.05);重症肺炎患儿SAA、cTnT、sTREM-1高于轻症肺炎患儿(P<0.05);Logistic回归分析显示,呼吸指数(RI)、氧合指数(OI)、新生儿危重评分(NCIS)、SAA、cTnT、sTREM-1是新生儿发生感染性肺炎的危险因素(P<0.05);ROC曲线显示,与SAA、cTnT、sTREM-1单项诊断相比,三项联合对新生儿感染性肺炎的诊断价值较高(P<0.05)。结论 SAA、cTnT、sTREM-1高表达于感染性肺炎新生儿,随着新生儿肺炎的加重,SAA、cTnT、sTREM-1表达逐渐升高,可用于对新生儿感染性肺炎的诊断。OBJECTIVE To analyze the changes in the expression of serum amyloid A(SAA),Troponin T(cTnT) and Soluble myeloid cell triggering receptor 1(STREM-1) in neonates with infectious pneumonia and their diagnostic value.METHODS Totally 200 newborns with infected pneumonia hospitalized in the neonatal department of the second Maternal and Child Health Hospital in Jinan from Jan.2020 to Jan.2022 were selected as the study group,and 150 healthy newborns during the same time were selected as the control group.The expression levels of SAA,cTnT and sTREM-1 were detected by enzyme-linked immunoassay,and the diagnostic value of SAA,cTnT and sTREM-1 on neonatal infectious pneumonia was analyzed by the receiver operating characteristic(ROC) curve.RESULTS The expressions of SAA,cTnT and STREM-1 were higher in the study group were higher than in the control group(P<0.05).The expressions of SAA,cTnT and STREM-1 were higher in children with severe pneumonia than in children with mild pneumonia(P<0.05).Logistic Regression analysis showed that respiratory index(RI),oxygenation index(OI),neonatal critical illness score(NCIS),SAA,cTnT,and sTREM-1 were risk factors for the development of infectious pneumonia in neonates(P<0.05);ROC curve showed that the combination of SAA,cTnT and STREM-1 was of higher diagnostic value in neonates with infectious pneumonia compared with SAA,cTnT and STREM-1 alone(P<0.05).CONCLUSION SAA,cTnT and STREM-1 were highly expressed in neonates with infectious pneumonia.With the aggravation of neonatal pneumonia,the expressions of SAA,cTnT and STREM-1 gradually increased,which could be used for the diagnosis of neonatal infectious pneumonia.
