发热门诊应对突发传染病疫情防控能力评价指标体系及模型的构建

摘　　要：目的 构建发热门诊应对突发传染病感染防控能力的评价指标体系,促进发热门诊规范化建设。方法 通过国家规范标准、文献查阅分析及头脑风暴法,初步构建评价指标体系。组织26位专家开展2轮德尔菲法专家咨询,确定评价指标体系。利用层次分析法确定各指标的权重。结果 形成了包括组织管理、位置与布局、工作流程、人员管理、通排风及空调、医疗废物与污水、资源储备、信息化设备8个一级指标和33个二级指标构成的发热门诊应对突发传染病感染防控能力评价指标体系。所有指标的重要性、可操作性平均得分均>4分,位置与布局(0.1638)、组织管理(0.1500)、人员管理(0.1379)指标的权重较大。利用各评价指标及权重确立评价模型。结论 发热门诊应对突发传染病感染防控能力评价指标及评价模型针对性强,结构合理,具有较高的可靠性和科学性,可为不断强化发热门诊应对突发传染病的感染防控能力建设提供科学依据。OBJECTIVE To construct an index system for evaluating the ability of fever clinics to deal with the prevention and control of infections from sudden-onset infectious disease,and to promote the standardized construction of fever clinics.METHODS An evaluation index system was initialized by means of national standards,literature review and analysis and brainstorming methods.Subsequently,26 experts were invited to carry out 2 rounds of Delphi method expert consultation to determine the evaluation index system.Each index in the system was weighted through the analytic hierarchy process.RESULTS An index system for evaluating the ability of fever clinics to deal with the prevention and control of infections from sudden-onset infectious disease comprised eight level-1 indexes including organizational management,location and layout,workflow,people management,ventilation and air conditioning,medical waste and sewage,resource reserves,information equipment,and 33 level-2 indexes.The average scoring of importance and operability of all the indexes was 4,with greater weighting for the indicators of location and layout(0.1638),organizational management(0.1500),and people management(0.1379).Each evaluation indicator and its weighting were used to establish the evaluation model.CONCLUSION The evaluation index system and evaluation model of fever clinics′ ability to prevent and control infections in response to outbreaks of infectious disease were highly targeted and reasonably structured,with high reliability and scientific validity,which could provide scientific evidence for continuously strengthening the construction of fever clinics′ ability to prevent and control infections in response to outbreaks of infectious disease.
