早期多元化肢体活动结合集束化护理对ICU机械通气患者神经肌肉功能及生活质量的影响

摘　　要：目的探讨早期多元化肢体活动结合集束化护理在重症监护病房(ICU)机械通气患者中的应用效果。方法选取2019年3月至2022年3月收治的80例ICU机械通气患者为研究对象,根据护理方法的不同将其分为对照组和观察组,每组40例。对照组采用常规护理,观察组在常规护理基础上应用早期多元化肢体活动结合集束化护理。比较两组的干预效果。结果干预后,观察组的功能独立性评定量表(FIM)各维度评分均高于对照组(P<0.05)。观察组的机械通气时间、ICU住院时间短于对照组,呼吸机相关性肺炎(VAP)、ICU获得性衰弱发生率低于对照组(P<0.05)。干预后,观察组的生活质量综合评定问卷(GQOLI-74)各维度评分高于对照组(P<0.05)。结论早期多元化肢体活动结合集束化护理有助于促进ICU机械通气患者神经肌肉功能改善,加快康复进程,降低VAP、ICU获得性衰弱发生风险,提高生活质量,值得推广。Objective To explore the application effect of early diversified physical activity combined with cluster nursing in intensive care unit(ICU)patients with mechanical ventilation.Methods Eighty ICU patients with mechanical ventilation admitted from March 2019 to March 2022 were selected as the study objects and divided into control group and observation group according to different nursing methods,with 40 cases in each group.The control group received routine nursing,and the observation group received early diversified physical activity combined with cluster nursing on the basis of routine nursing.The intervention effects of the two groups were compared.Results After intervention,the each dimension scores of Functional Independent Measure(FIM)of the observation group were higher than those of the control group(P<0.05).The mechanical ventilation time and ICU stay time in the observation group were shorter than those in the control group,and the incidences of ventilator associated pneumonia(VAP)and ICU acquired asthenia in the observation group were lower than those in the control group(P<0.05).After intervention,the each dimension scores of Generic Quality of Life Inventory-74(GQOLI-74)in the observation group were higher than those in the control group(P<0.05).Conclusion Early diversified physical activity combined with cluster nursing can promote the improvement of neuromuscular function in ICU patients with mechanical ventilation,accelerate the rehabilitation process,reduce the risk of VAP and ICU acquired asthenia,and improve the quality of life,which is worth popularizing.
