外周血淋巴细胞计数在血氧饱和度下降的新型冠状病毒感染患者中的应用

摘  要:目的回顾性比较50例血氧饱和度下降的新型冠状病毒感染(COVID-19)患者和50例血氧饱和度正常的COVID-19患者外周血淋巴细胞计数、中性粒细胞/淋巴细胞比值(neutrophil/lymphocyte ratio,NLR)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)水平和肺部影像资料,并分析COVID-19患者外周血淋巴细胞计数、NLR、hs-CRP水平、肺部影像学资料与发生血氧饱和度下降的关系。方法选取50例血氧饱和度下降的COVID-19患者(血氧饱和度下降组)和50例血氧饱和度正常的COVID-19患者(血氧饱和度正常组),各组所有纳入对象均进行外周血淋巴细胞计数、NLR计算、hs-CRP水平检测,分析血氧饱和度下降组和血氧饱和度正常组的影像学资料。结果血氧饱和度下降组NLR与血氧饱和度正常组相比差异无统计学意义;血氧饱和度下降组淋巴细胞百分率(percentage of lymphocyte,LYM%)显著低于血氧饱和度正常组;血氧饱和度下降组hs-CRP水平显著高于血氧饱和度正常组;血氧饱和度下降组肺部感染率显著高于血氧饱和度正常组。结论COVID-19患者外周血淋巴细胞降低可能是炎症更加严重和更加容易发生肺部感染的直接原因,进而更容易引起患者血氧饱和度下降。Objective To retrospective comparison of 50 cases of oxygen desaturation coronavirus disease(COVID-19)patients and 50 cases of blood oxygen saturation normal COVID-19 patients peripheral blood lymphocyte count,neutrophil/lymphocyte ratio(NLR),hypersensitive C-reactive protein(hs-CRP)levels and pulmonary image data,19 patients peripheral blood lymphocyte count and analyze COVID-19,NLR,hs-CRP levels,lung imaging data with oxygen desaturation of relationship.Methods Select 50 cases of oxygen desaturation COVID-19 patients(oxygen desaturation group)and 50 cases of blood oxygen saturation normal COVID-19 patients(blood oxygen saturation normal group),each group all included in the object of peripheral blood lymphocyte count,NLR calculation,hs-CRP level detection,Imaging data of decreasing oxygen saturation group and normal oxygen saturation group were analyzed.Results There was no significant difference in NLR between the decreased oxygen saturation group and the normal oxygen saturation group.The percentage of lymphocyte(LYM%)in decreased oxygen saturation group was significantly lower than that in normal oxygen saturation group.The level of hs-CRP in the decreased oxygen saturation group was significantly higher than that in the normal oxygen saturation group.The lung infection rate in the decreased oxygen saturation group was significantly higher than that in the normal oxygen saturation group.Conclusion COVID-19 patients peripheral blood lymphocyte to reduce inflammation may be more serious and more prone to the immediate cause of lung infection,and were more likely to lead to patients with oxygen desaturation.
