理气健脾方治疗脾虚气滞型功能性消化不良患者停用西药后症状反跳的临床研究

摘　　要：目的探讨理气健脾方治疗脾虚气滞型功能性消化不良患者停用促胃肠动力药、质子泵抑制剂(proton pump inhibitor,PPI)后症状反跳的临床疗效,以期为临床治疗该疾病提供借鉴。方法收集2022年2月至10月,61例理气健脾方治疗停用促胃肠动力药、PPI后症状反跳的功能性消化不良患者的临床资料,所有患者皆排除幽门螺杆菌感染。比较理气健脾方治疗前,治疗第2周、第4周患者主要临床症状分布、中医症状评分、生活质量评分,治疗效果及记录不良反应情况。结果共纳入符合要求的功能性消化不良患者61例。治疗第4周总有效率为93%,高于第2周总有效率为82%,差异有统计学意义(P<0.05);治疗第2周、第4周患者主、次症评分低于治疗前,症状明显改善,差异有统计学意义(P<0.05);治疗第4周与治疗第2周相比患者胃脘痞满、胃脘胀痛、纳呆、嗳气症状评分均降低,差异有统计学意义(P<0.05);疲乏、便溏症状评分无明显变化,差异无统计学意义(P>0.05)。与治疗前比较,治疗第2周、第4周各患者症状总积分均有明显改善,差异有统计学意义(P<0.05)。治疗期间未发生不良反应及事件。结论理气健脾方对脾虚气滞型功能性消化不良患者停用促胃肠动力药、PPI后症状反跳具有较好的临床疗效,可以显著改善临床症状,提高患者生活质量且安全性高。Objective To investigate the clinical efficacy of LiqiJianpi decoction in the treatment of spleen deficiency and qi stagnation type functional dyspepsia patients with symptom rebound after discontinuation of gastrointestinal motility drugs and proton pump inhibitor(PPI),with a view to providing reference for the clinical treatment of this disease.Methods From February 2022 to October 2022,clinical data were collected from 61 patients with functional dyspepsia treated with LiqiJianpi decoction after discontinuation of gastrointestinal stimulants and proton pump inhibitors,and all patients were excluded from H.pylori infection.To compare the distribution of main clinical symptoms,TCM symptom scores,quality of life scores,treatment efficacy and recording of adverse reactions in patients before,at week 2 and at week 4 of the treatment with LiqiJianpi decoction.Results A total of 61 patients with functional dyspepsia meeting the requirements were included.The total effective rate was 93%at week 4 of treatment,which was higher than the total effective rate of 82%at week 2,and the difference was statistically significant(P<0.05).Patients’all symptom scores were lower than before treatment at weeks 2 and 4,and symptoms improved significantly,with statistically significant differences(P<0.05).Compared with the second week of treatment,the scores of gastric fullness,gastric distension,nausea and belching decreased in the fourth week of treatment,and the difference was statistically significant(P<0.05);the scores of fatigue and loose stools did not change significantly,and the difference was not statistically significant(P>0.05).Compared with the pre-treatment period,the total symptom scores of each patient improved significantly at the 2nd and 4th weeks of treatment,with statistically significant differences(P<0.05).No adverse reactions or events occurred during the treatment period.Conclusion LiqiJianpi decoction has good clinical efficacy on the rebound of symptoms in patients with functional dyspepsia with spleen defi
