糖尿病肾脏病中医证型与炎症因子的相关性研究

摘　　要：目的探讨糖尿病肾脏病(Diabetic kidney disease,DKD)气阴两虚证、湿热证、血瘀证与脂代谢、炎症因子的相关性。方法选取DKD患者300例作为研究组(气阴两虚证114例、湿热证93例、血瘀证93例),选取30例健康体检者作为正常对照组。分别记录中医证候、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平。通过统计学分析,比较三证型患者的炎症因子指标变化,探讨其相关性。结果与正常对照组比较,三证型患者的IL-6、hs-CRP、TNF-α水平更高,差异有统计学意义(P<0.05)。湿热证评分与IL-6水平呈正相关,与hs-CRP、TNF-α水平呈负相关(P<0.05),血瘀证评分与TNF-α呈正相关(P<0.05)。结论IL-6、hs-CRP可能与湿热证存在相关性,TNF-α可能与湿热证及血瘀证存在相关性。Objective To investigate the relationship between qi-yin deficiency syndrome,dampness-heat syndrome,blood stasis syn-drome and lipid metabolism and inflammatory factors in diabetic kidney disease(DKD).Methods A total of 300 DKD patients were se-lected as the study group(114 cases of qi-yin deficiency syndrome,93 cases of dampness-heat syndrome,93 cases of blood stasis syn-drome),and 30 healthy subjects were selected as the normal control group.TCM syndromes,interleukin-6(IL-6),hypersensitive C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)levels were recorded respectively.Through statistical analysis,the changes of inflammatory factors in patients with three syndrome types were compared and their correlation was discussed.Results Compared with the normal control group,the levels of IL-6,hs-CRP and TNF-αwere higher in patients with three syndrome types,and the difference was statistically significant(P<0.05).Dampness-heat syndrome score was positively correlated with IL-6 level,negatively correlated with hs-CRP and TNF-αlevel(P<0.05),blood stasis syndrome score was positively correlated with TNF-αlevel(P<0.05).Conclusion IL-6 and hs-CRP may be correlated with dampness-heat syndrome,TNF-αmay be correlated with dampness-heat syndrome and blood stasis syndrome.
