四黄汤在预防骨科静脉炎中的应用

摘　　要：目的探究中药四黄汤预防骨科患者发生静脉炎的应用效果。方法选取厦门市中医院2019年1月—2021年3月收治的86例骨折术后患者,按随机数字表法分成2组。对照组接受透明水胶体敷料贴敷防治,观察组于对照组基础上联合应用中药四黄汤防治。对比2组防治7 d后静脉炎发生情况;防治前,防治3 d、7 d后视觉模拟量表(VAS)评分;防治前、防治7 d后血管内皮功能指标、凝血指标水平。结果防治7 d后,观察组静脉炎发生情况优于对照组(P<0.05);防治3 d、7 d后,观察组VAS评分均低于对照组(P<0.05);防治7 d后,观察组血清内皮素-1(ET-1)、细胞黏附分子-1(ICAM-1)、血管内皮生长因子(VEGF)水平均低于对照组(P<0.05);防治7 d后,观察组血清血栓素B2(TXB2)水平低于对照组,血清6-酮-前列环素F1α(6-keto-PGF1α)水平高于对照组(P<0.05)。结论中药四黄汤可有效降低骨科患者静脉炎发生风险,减轻机体疼痛感,提高机体凝血功能,改善血管内皮功能。Objective To explore the effect of Sihuang decoction on prevention of phlebitis in orthopedic patients.Methods A total of 86 patients after fracture surgery in Xiamen Hospital of Traditional Chinese Medicine from January 2019 to March 2021 were selected and divided into two groups according to random number table method.The control group was treated with transparent hydrocolloid dressing,and the observation group was treated with Sihuang decoction.The incidence,visual analog scale(VAS)scores,vascular en⁃dothelial function indexes and coagulation indexes before and after 7 days of phlebitis treatment were compared between the two groups.Results After 7 days of prevention and treatment,the occurrence of phlebitis in the observation group was better than that in the control group(P<0.05).After 3 and 7 days of prevention and treatment,VAS scores in observation group were lower than those in control group(P<0.05).After 7 days of prevention and treatment,the serum levels of endothelin-1(ET-1),cell adhesion molecule-1(ICAM-1)and vascular endothelial growth factor(VEGF)in observation group were lower than those in control group(P<0.05).After 7 days of prevention and treatment,the serum thromboxane B2(TXB2)level in observation group was lower than that in control group,and the serum 6-keto-PGF1α(6-Keto-PGF1α)level in control group was higher than that in control group(P<0.05).Conclusion Si⁃huang decoction can effectively reduce the risk of phlebitis in orthopedic patients,relieve pain,improve coagulation function and im⁃prove vascular endothelial function.
