血清NSE、CYFRA21-1、PSA、β2-MG、TAP联合检测在前列腺癌早期诊断中的效能分析

摘　　要：目的探讨血清神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)、前列腺特异抗原(PSA)、β2微球蛋白(β2-MG)、肿瘤异常蛋白(TAP)联合检测诊断前列腺癌的临床价值。方法选取我院2020年1月至2022年12月收治的前列腺癌患者120例作观察组,另选良性前列腺疾病患者120例作对照组。所有研究对象均采血检测NSE、CYFRA21-1、PSA、β2-MG、TAP水平,比较两组间上述指标差异,并比较不同分期前列腺癌患者上述指标差异。绘制ROC曲线分析血清NSE、CYFRA21-1、PSA、β2-MG、TAP及联合检测诊断前列腺癌的临床价值。结果观察组血清NSE、CYFRA21-1、PSA、β2-MG、TAP水平高于对照组,差异有统计学意义(P<0.05);Ⅰ~Ⅱ期前列腺癌患者血清NSE、CYFRA21-1、PSA、β2-MG、TAP水平低于Ⅲ~Ⅳ期患者,差异有统计学意义(P<0.05);ROC曲线显示,血清NSE、CYFRA21-1、PSA、β2-MG、TAP及联合检测诊断前列腺癌的曲线下面积分别为:0.741、0.789、0.743、0.755、0.775、0.924。结论血清NSE、CYFRA21-1、PSA、β2-MG、TAP在前列腺癌早期诊断中均具有一定价值,联合检测价值更高,有助于减少漏诊、误诊发生。Objectives To investigate the clinical value of combined detection of serum neuron specific enolase(NSE),cytokeratin 19 fragment(CYFRA21-1),prostate specific antigen(PSA),β2 microglobulin(β2-MG)and tumor abnormal protein(TAP)in the diagnosis of prostate cancer.Methods A total of 120 cases of patients with prostate cancer admitted to our hospital from January 2020 to December 2022 were selected as the observation group,and 120 patients with benign prostate disease were selected as the control group.The levels of NSE,CYFRA21-1,PSA,β2-MG and TAP were detected in all subjects.The differences of the above indexes between the two groups were compared,and the differences of the above indexes in patients at different stages of prostate cancer were compared.ROC curve was drawn to analyze the clinical value of serum NSE,CYFRA21-1,PSA,β2-MG,TAP and combined detection in the diagnosis of prostate cancer.Results The levels of serum NSE,CYFRA21-1,PSA,β2-MG and TAP in the observation group were significantly higher than those in the control group(P<0.05).The levels of serum NSE,CYFRA21-1,PSA,β2-MG and TAP in patients with stage I-II prostate cancer were significantly lower than those in patients with stage III-IV(P<0.05).ROC curve showed that the area under the curve of serum NSE,CYFRA21-1,PSA,β2-MG,TAP and combined detection in the diagnosis of prostate cancer was 0.741,0.789,0.743,0.755,0.775 and 0.924,respectively.Conclusions Serum NSE,CYFRA21-1,PSA,β2-MG and TAP are of certain value in the early diagnosis of prostate cancer.Combined test can further improve the diagnostic efficiency,which is helpful to reduce the occurrence of missed diagnosis and misdiagnosis.
